Be an Abundant Life Aging Life Care™ Manager (Certified or Certification-Eligible)

Overview:
We are seeking a dedicated, credentialed, or credential-eligible Aging Life Care™ Manager to join our team. This role is ideal for a professional passionate about supporting older adults and individuals facing health and life challenges through a holistic, client-centered approach. Our Care Managers serve as advocates, navigators, and strategic partners for clients and their families—helping them make informed decisions and reduce caregiver stress while enhancing quality of life.

About the Role:
As an Aging Life Care Manager, you will coordinate and oversee care for older adults and those with complex needs. You will work independently while also collaborating closely with families, providers, and community resources. Your work will span clinical, psychosocial, financial, and environmental domains, and will often include mediation, education, and long-term planning.

Primary Responsibilities:

Conduct comprehensive biopsychosocial assessments to identify client needs, preferences, and goals.
Develop, implement, and adjust individualized care plans.
Provide family education, coaching, and conflict mediation to support informed, compassionate decision-making.
Coordinate healthcare, housing, financial, legal, and in-home support resources.
Serve as the primary point of contact and care advocate for clients and families.
Collaborate with multidisciplinary professionals including physicians, attorneys, fiduciaries, and housing providers.
Navigate crisis situations and facilitate transitions of care (e.g., hospital to home, or to higher levels of care).
Maintain professional documentation and meet ethical and professional standards as guided by ALCA.

Required Qualifications:

Bachelor’s degree in nursing, social work, gerontology, psychology, or a related field (Master’s degree preferred).
Minimum of two years’ experience in care management, social work, nursing, geriatric care, or a related profession.
Meets requirements for Aging Life Care Association membership.
Holds (or is actively working toward) an ALCA-approved certification such as:
Certified Care Manager (CCM)
Certified Case Manager (CCM)
Licensed Clinical Social Worker (LCSW)
Registered Nurse (RN)
Licensed Professional Counselor (LPC)
Advanced Aging Life Care Professional™ credential (preferred but not required)

Ideal Candidate Attributes:

Strong communication, conflict resolution, and advocacy skills
Familiarity with Positive Behavior Support™ and trauma-informed care approaches a plus
