Who is Bridging Access to Care, Inc.: Bridging Access to Care, Inc. (BAC) is a nonprofit, multi-service, community-based behavioral health care organization committed to building healthy communities, one person at a time.

In 2012, Brooklyn AIDS Task Force changed its name to BAC and successfully transitioned from an AIDS-centered organization to a multi-service, community-based healthcare organization. Brooklyn Aids Task Force was the very first HIV/AIDS organization in Brooklyn prioritizing serving persons living with HIV/AIDS (PLWHA) since 1986. Today, BAC provides mental health and substance use-related social services, comprehensive HIV/AIDS care, and coordinated health care and supportive services to underserved racial and ethnic minority communities in Brooklyn and other parts of New York City.

JOB SUMMARY: We are seeking a Bilingual and Non- Bilingual Care Manager. The Health Home Care Manager is responsible for providing comprehensive care coordination, advocacy, referrals, and crisis intervention for individuals enrolled in the Health Home program. Serving as the primary point of contact, the Care Manager works collaboratively with healthcare providers, community agencies, and support networks to address members' medical, behavioral health, and social service needs.

This role develops and monitors individualized care plans, conducts comprehensive assessments, facilitates access to community resources, and ensures continuity of care to improve health outcomes and quality of life. The Care Manager maintains accurate documentation, ensures compliance with Health Home, Department of Health (DOH), HIPAA, and agency standards, and conducts home visits and accompanies members to appointments as needed. This position requires strong case management skills, sound clinical judgment, excellent communication, and the ability to build effective relationships while managing a caseload of up to 50 members. Travel of up to 75% is required.

EDUCATION AND WORK EXPERIENCE:

MINIMUM: Bachelor’s degree in psychology, Sociology, Social Work, or another related field.
MINIMUM: Two (2) years’ experience working in the case management field or setting providing direct services to persons with Serious Mental Illness, HIV, homelessness, or substance use disorder, or Linking individuals with Chronic Illnesses (including HIV/AIDS), Serious Mental Illness (SMI) or Serious Emotional Disturbance (SED) to a broad range of services essential to successful living in a community setting (e.g., medical, psychiatric, social, educational, legal, housing and/or financial services)
Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
Up to 75% of travel is required.

LANGUAGE: Bilingual/Spanish speaking required. Must be able to read, write, speak, and comprehend Spanish and English.

DUTIES AND RESPONSIBILITIES:

Care Management & Service Provision

Provide monthly care management to up to 50 Medicaid users.
Conduct case conferences with service providers to ensure effective care coordination.
Make referrals for additional services as requested or needed by members.
Ensure all services meet DOH Health Home and BAC standards.
Coordinate patient care plans in collaboration with healthcare teams, ensuring continuity of care across different settings.
Follow-Ups: Conduct regular follow-ups to assess progress, identify additional needs, and update service plans accordingly.

Field Work

Conduct home visits and accompany members to medical and mental health appointments.
Up to 75% of travel is required.

Documentation & Compliance

Co-document all services and case conferences in the State and internal databases within specified timeframes.
Maintain accurate, up-to-date records of all assigned members.
Create and manage member charts, including required consents and forms.
Submit monthly HMLs (Health Management Logs).
Ensure compliance with HIPAA regulations regarding patient confidentiality and medical records management.

Assessments & Plans of Care

Complete comprehensive assessments and reassessments for all members.
Develop and regularly update Plans of Care in line with State mandates.
Acquire necessary consents for service provision and communication with other providers.

Meetings & Trainings

Participate in weekly supervision and team meetings.
Attending in-house and off-site training.
Serve on agency committees as assigned.

Additional Responsibilities

Perform other duties as required by the needs of the program and BAC requirements.

KNOWLEDGE, SKILLS, AND ABILITIES:

Health Home guidelines and procedures
Possesses excellent judgment and the ability to maintain professional relationships and boundaries when working with clients and peers
Strong knowledge of acute care, outpatient, inpatient, and managed care environments.
Comfortable with digital tools and software for project management, data tracking, and communication, with a commitment to leveraging technology to improve program outcomes
Proficient in collecting, analyzing, and interpreting data to inform program decisions and demonstrate impact.
Excellent verbal and written communication skills, including the ability to convey complex information clearly and sensitively to diverse audiences.
Strong understanding of and sensitivity to the diverse backgrounds, experiences, and needs of individuals living with HIV, including those from marginalized communities.

Salary will be based on, but not limited to, the candidate’s education, experience, and skills.
