Full Job Description

Wraparound Facilitator
Hope Enacted | Colorado (Statewide)
Work Structure: ~80–90% Remote | ~10–20% In-Home & Community
Compensation: $40/hour (billable and non-billable time)
Must be a current Colorado resident

About the Role

This is a relationship-driven role working directly with youth and families navigating complex emotional, behavioral, and system-related challenges through Colorado's CHRP Medicaid waiver program.

This is not task-based work or a traditional case management role. You'll be building real relationships with families over time, helping them create a team around them, and following through on what actually moves things forward. Facilitators typically work with a family for 1–2 years per case.

No prior wraparound certification is required — we provide training and support you through the credentialing process.

Hope Enacted is a Colorado-based wraparound facilitation agency delivering services statewide. We are focused on relationship-driven, individualized care that produces long-term outcomes for families. We are a small, intentional team and we intend to keep it that way.

What You'll Be Doing

Facilitate wraparound team meetings with families, providers, and natural supports
Implement the four phases of the wraparound model
Help families define goals and build practical, working plans
Navigate systems — schools, services, community supports
Maintain consistent communication across all team members
Support families during difficult or high-stress situations
Track progress and complete documentation
Research and connect youth to additional services and resources
Meet families in homes or community settings when appropriate

Schedule & Caseload

Ramp-Up Timeline

Weeks 1–5: Onboarding and initial families (20–30 hours/week)
Week 5+: Building toward full workload (~40 hours/week)

Caseload Structure

Early: 3–5 families (more time-intensive at the start)
Long-term: ~12–15 families as things stabilize

Schedule

Self-managed and flexible
Some evening availability encouraged based on family need
Requires strong organization and consistent follow-through

Remote & In-Person Expectations

The majority of this work is remote. You must have reliable internet, a consistently private and professional workspace, and your own computer and phone. All necessary accounts, logins, and a Google Voice number will be provided.

Approximately 10–20% of the role is in-person — home visits and community-based meetings. Frequency depends on family need. Mileage reimbursement is provided for approved travel at $0.25/mile.

Support & Supervision

Facilitators receive ongoing supervision and support directly from the agency director. Case referrals and administrative work are handled on your behalf so you can stay focused on families.

1099 Structure

This is a 1099 Independent Contractor position — not a W-2 employee role. You are responsible for your own taxes and benefits and manage your own schedule within mutually agreed-upon expectations.

This is not short-term contract work. We are building a team of facilitators who want to be here long-term and grow with the organization. Work volume depends on caseload and engagement — facilitators who commit fully typically build to 40 hours/week within 5–6 weeks. There are no absolute guarantees of hours or income.

This role offers flexibility, but it requires accountability.

Basic Qualifications

Bachelor's degree or higher in social work, psychology, human services, or a related field
Experience working with youth and families
Ability to work independently and manage your own schedule
Current Colorado resident
Must pass a background check prior to starting
