Director, RN Case Manager for Medicaid Home Health.  Communicated with RN's, LVN's about schedule and any changes patient's needed to change.  Reviewed all paperwork and time sheets from previous visits made in the last 48 hrs.  Patient's with any issues were called so that resolution to any problem could be solved ASAP.  Communicated with insurance company, Discharge planner, RN Navigator, RN Case Manager Admit nurse, reviewed all referrals for Medicaid criteria, report to 30 day monitoring nurse.  Worked with onsite team and insurance team to ensure best care for patient. Reviewed all new PCP orders, PT, OT, and Speech therapy notes.  Obtained all authorizations with proper codes, Communicated with Administrator about patients.  Any hospitalizations of patients, hospital called for progress, estimate of discharge.  Triaged all patient calls.  Admission of patients.  Developed inidividualized plan of care for patient meeting all healthcare and holistic needs.  Referrals as needed.  Supervisory visits, field visits as needed. Billing, coding, ordered supplies.  Spreadsheet for all patients with status, date of admit, goals, teaching, and estimated time patient may reach all goals.  QA, UR of all documentation and charts.  Communicated with nurses about changes needed, trained for improved documentation, ensured plan of care be followed.  Audits of charts for proper forms, filed accurately.  Evaluations of nurses every 3 months.  Discussed with each nurse individually for improvement of skills, documentation.  Case conference scheduled, prepared all documents needed, worked with onsite team on any problems or issues patient experiencing.  Continued communication with Dr.'s, staff.  Monitored all orders received in a timely manner.

management is ungrateful and wholly unorganized. tasks were almost always duplicated. training is dubious to nonexistent. on numerous occasions i felt as though i was informing my managers of some info about the company that they themselves didn't know. they pressure you for suggestions on how to make tasks more efficient and streamline, only to reject every offer you make. the main director travels most of the time and is completely inaccessible, but if you don't reach her she will tell you it is your fault. the work product is questionable. they keep the office understaffed and overworked, so quality control is nonexistent; they don't even have the time to review work themselves. the director is incredible micromanaging, even from afar. she demands staff keep task lists and "own" their work, however you are required to review your daily to-do list every morning and in between each task with her or her 2nd. it makes working in a timely efficient manner impossible. there are other aspects of the behavior and management style that are extremely questionable, borderline fraudulent. the manager and 2nd make more excuses for their lack of capabilities and abuse staff as scapegoats for their own errors more than any two people i have ever known. i have even heard them trash talk each other behind each others' backs. the environment is very negative and very toxic. the turnover rate is the highest i have ever seen, counting 5 people in one admin asst position since jan 2012!!!! you have been warned.

A typical day was managing the day to day operation of a long term care facility.  Would ensure that all attendants reported for work and provide quality work to our patients.  Ensuring that services were correctly charged.  Ensuring that all federal and state regulations were being followed.  

Learn to appreciate what I have because the patients we took care of were very poor, lacked family support and sometime lived in terrible conditions.  

My co-workers varied in work ethic and personalities.  As a manager I needed to find out how to get them to be the best they could.  

The hardest part of my job was keeping attendants.  They did not like the job and many times told me they made more on welfare than working as an attendant.  

The best part of the job was at Christmas when we would go to an orphanage in Juarez and give them a Christmas party.  All teammates dressing up a Christmas characters.

In a typical day I come in at 7 am to open up the clinic, run our daily state reports, as well as our lab.  I check the messages left and cross reference them in our system to see if they have gotten benefits, and what they need to come in for.  As clients begin to show up, I check them in and see what kind of appointment they are there for so I could ask for the proper information we need.  Once I clarify what they are there for I gather the documents needed to begin the paperwork.  All our paperwork are state documents so you need to make sure everything is filled out correctly.  Once that is done I take the client to the lab to assist in getting the client's height/weight and iron checked.  We see at least 30 clients a day, I also help translate because I my supervisor does not speak Spanish. In between clients I also answer phone calls and reschedule appointments. At the end of the day we shut everything down, clean and lock up.

After my husband died of a long illness and I'd taken care of him during this time - I decided to go into this field.  I enjoy working with people and I enjoy this position very much.  The elderly lady I took care of could get around pretty good and we enjoyed our times together.  I cleaned, cooked and assisted her with her needs.  I would drive her to doctor appointment, take her shopping, and we'd have lunch out some times.  She and I had a good time together and I met her needs.  

Upon arrival I'd check her the start fixing her breakfast.  I clean the house during the day, do dishes and laundry.  Keep her company while she watch TV.  Monitored her bath to ensure she didn't fall in the shower.  I'd enter act with nurses and insure her medicine was there for review and I'd give them updates as needed.  Before I'd leave for the evening I'd ensure she had what she needed with reach.

I have learned that this company is very shady, you can never get a direct answer about your pay rates, and you are always told that your checks balance out??? I have been shorted almost 50$ in mileage, I have been shorted almost 30 work hrs from the day I started till now.

I believe they have NON medical people doing stuff they have no business doing for clients. Stuff that only a medical CNA, Registered Nurse should be doing. If you bring it up to them they say it's a case by case situation.

They have had many clients fire the company since I have been there. The employee turn around rate is high.

A typical day for me, I start my day when I leave my house. So I am usually gone by 6:30 or 7am, I drive all over from Dublin, Tx to Grapevine, Tx. I put many many miles and wear and tear on my car. All for 8.50 an hr. I cook, clean, bathe, and shop for disabled and elderly. I can be home by 5:30 or as late as 11:30pm depending on where a client is.

Originally, I was informed that my shifts start at 7AM and end sometime in the afternoon, or evening at the latest. However, I got off around midnight or past midnight most of the time. My employer would clock me out a couple of hours before my shift is over, I had a flu and my employer told me that that is not an excuse for me to miss work. She would complain to me about all of her other co-workers and I would definitely not recommend working here. I gave a phone call about receiving a document, and was blamed for not letting me know as soon as I moved. Terrible service, my co-workers definitely did not feel appreciated being there and quit as soon as they could. Other companies would pay at least double the amount to do the job we had to do, yet we were paid minimum wage, $7.25/hr. I have been called in the middle of the night at 1AM to come to work.

The management at Outreach is trash to say the least. This is an understatement in fact. There are positions to move up within the company but are essentially already assigned to those that supervisors favor. The supervisors are idiots and will most likely go no further than outreach and retire making only $40K a year. But they are fine with that, this shows the lack of ambition and complexity of the individuals working there. There is no dress code. Despite enforcement attempts. The turnover rate is much too high.The CEO of the company left within a year there. So that alone reflects the instability. I enjoyed my tasks here as an employee, but knowing what I know now, I probably would not have accepted the job. If you like to be surrounded by fecal matter on legs, then this is the position for you. They are nothing short of a ghetto call center in the sound  is no dress code desl

A typical day at work consisted of uploading documents, data entry, copy, scanning, and quality check paperwork.  Answering phones, customer service, and payroll/billing.  I did insurance verification for Amerigroup and Superior once a month, and verified eligibility weekly.  I worked in the EVV system which was State Mandated on a daily basis.
The hardest part of my job was keeping in compliance with the State Mandated EVV system as we had to maintain this for 400 client/attendants.

The most enjoyable part of my job was working with my coworkers and working with the client's, I really enjoyed that.

The workplace culture was really good for the most part and we all got along really good and worked very well together.  I miss them..

Completely disorganized. Impossible to get a straight answer out of management about even the most mundane things, and then they blame you if things go wrong. Constantly made arbitrary changes that never improved anything and just caused unnecessary upheaval. NO actual training; they essentially just expect you to sit and watch someone else do the job and take notes. I never got all the equipment or access I needed to do my job and was constantly told they were "working on it." Violated HIPAA by letting untrained people view sensitive client medical information long after any reasonable time frame after being hired. One of the managers would make inappropriate comments to female employees. The work itself was pretty rewarding but the chaos wasn't worth it.
