Full Job Description

Overview

The Case Manager (LPN – Bilingual Spanish) is responsible for coordinating patient care from referral through case completion. This role serves as the primary point of contact for patients, families, healthcare facilities, physicians, insurance companies, assistance companies, and internal teams to ensure patients receive safe, timely, and compassionate care.

The ideal candidate is a licensed practical nurse (LPN) who is fluent in both English and Spanish and possesses excellent communication, organizational, and critical thinking skills. This position requires the ability to manage multiple active cases while maintaining a high level of customer service and professionalism.

Essential Duties and Responsibilities

Case Management

Coordinate patient cases from intake through completion.
Review medical records to assess patient needs and identify potential barriers to care.
Collaborate with physicians, hospitals, case managers, insurance companies, and assistance providers regarding patient care plans.
Monitor active cases and communicate changes in patient status to appropriate stakeholders.
Maintain timely follow-up with patients, families, healthcare providers, and clients.
Ensure all required documentation is complete, accurate, and submitted in a timely manner.

Patient & Family Communication

Serve as the primary point of contact for patients and their families.
Explain care plans, services, and next steps in a compassionate and professional manner.
Provide updates throughout the coordination process.
Translate medical and care-related information between English and Spanish as needed.

Clinical Coordination

Coordinate with physicians, nurses, and other healthcare professionals to facilitate continuity of care.
Review physician orders and medical documentation for completeness.
Assist in identifying patient care needs and escalating concerns to the appropriate medical professionals.
Monitor patient progress and communicate significant changes to the care team.

Documentation & Compliance

Maintain detailed and accurate case documentation.
Ensure compliance with HIPAA and all company confidentiality policies.
Complete case notes, reports, and required documentation according to company standards.
Maintain accurate electronic medical records and client documentation.

Client Relations

Communicate professionally with hospitals, insurance companies, assistance companies, physicians, and referral sources.
Respond promptly to inquiries regarding active cases.
Maintain a high level of customer service while managing sensitive and complex situations.
Build positive working relationships with clients and healthcare partners.

Required Qualifications

Current Licensed Practical Nurse (LPN) license in good standing.
Fluent in English and Spanish (written and verbal).
Minimum of two (2) years of clinical nursing experience.
Excellent verbal and written communication skills.
Strong organizational and multitasking abilities.
Ability to prioritize multiple cases in a fast-paced environment.
Proficiency with Microsoft Office and electronic medical record (EMR) systems.
Strong critical thinking and problem-solving skills.
Ability to work independently and collaboratively within a multidisciplinary team.

Preferred Qualifications

Experience in case management, care coordination, or utilization review.
Experience working with insurance companies or healthcare assistance organizations.
Knowledge of medical terminology and clinical documentation requirements.
Experience in emergency medicine, medical-surgical nursing, home health, or care management.
Bilingual customer service experience in a healthcare setting.

Compensation

Pay:$25.00–$35.00 per hour, based on experience, qualifications, and relevant clinical background.
