Full Job Description

Job Summary

Under the direct supervision of the Child and Family Connections Program Manager, the Service Coordinator will assist families in navigating the Early Intervention system through intake, Individualized Family Service Plan (IFSP) development, and transition at age 3 years; will link children and families to the necessary services and supports; will serve as a liaison between Early Intervention services for families and community resources; will work cooperatively with CFC staff, EI providers and school district personnel. The Child and Family Connections Service Coordinator will have significant travel throughout McHenry County, with periodic out of county meetings/trainings.

This is a grant funded position, the duties and/or requirements of the position may be modified as required by the funding source.

Work Hours: 37.5 hours per week (1.0 FTE) or unless otherwise specified in individual personnel file; Occasional night and/or weekend work

Essential Functions

· Conduct a comprehensive intake with families which includes developmental and social/emotional screenings

· Convene and facilitate Individualized Family Service Plan (IFSP) meetings

· Ensure compliance with Early Intervention protocols

· Utilize Cornerstone database for all aspects of Early Intervention data entry, including casenotes. Effectively and accurately enter authorizations for therapists to provide services.

· Utilize Microsoft office programs effectively and accurately, including Outlook, Word, Access and Excel

· Maintain accurate and timely entries into client record and Cornerstone System

· Maintain accurate and timely entries into Access database

· Maintain client records in compliance with accreditations and funder requirements

· Maintain strict confidentiality in all aspects of work, including but not limited to computer/laptop security, conversations regarding clients, and client file security. Strict adherence to Options & Advocacy computer, electronic and confidentiality policies

· Provide accurate accounting of time and expenses

· Maintain regular contact with all families: at a minimum of monthly for all ongoing families, a minimum of weekly for all families waiting for services to provide ongoing updates on their service provision

· Effectively transition families/children using required timelines out of Early Intervention into applicable services at age three, including local school districts, Head Start, appropriate departments within Options & Advocacy and other community resources

· Develop a collaborative relationship with families to follow-up and monitor that family’s needs are being met

· Actively participate in quality improvement processes as applicable to the position

· Understand and support the mission, vision and values of Options and Advocacy

Organizational Relationships

Supervised by: Child and Family Connections Assistant Program Manager

Supervises: N/A

Internal: All departmental personnel that are employed by Options and Advocacy

External: Clients, families, Early Intervention therapists, school districts, general public, regulatory and licensing agencies, county, state and federal representatives, and all organizations that interact with Options and Advocacy.

Professional Background and Experience

Minimum of a Bachelor’s Degree in a related Human Services field

· Knowledge and understanding of Early Intervention

· One year of specialized training or experience in the field of developmental disabilities preferred

· Experience with children 0-3 years old and families preferred

· Successful credentialing through the State of Illinois Early Intervention system. After hire, completion of 120 hours of approved training during first 18 months, 30 hours of approved training during each subsequent 3 year period

· Successful completion of a criminal background check, including but not limited to: fingerprinting, the Child Abuse and Neglect Tracking system (CANTS) and state police background check

Skills and Capabilities

· Ability to work cooperatively within a multi-disciplinary, multi-agency team approach that also includes parents, schools, and other community members and agencies

· Ability to communicate clearly and professionally, both orally and in writing

· Knowledge and understanding of Early Intervention Philosophy and Principles and the ability to adhere to these principles in practice

· Ability to work independently, maintaining accurate accounting of caseload and meeting requirements for each family

· Ability to understand, interpret and explain a complex state system of rules, principles and guidelines and ensure that all aspects of those rules are integrated into your work

· Excellent organizational skills and the ability to successfully manage a caseload of 50-90 clients

· Knowledge of early child development and the ability to recognize atypical development

· Possess a strong commitment to child and family centered services and excellent communication skills, including the ability to use conflict resolution techniques

· Ability to make and maintain personal work schedule using Outlook to accurately identify schedule and locations of meetings to ensure the completion of work as well as the safety of all employees

· Possess a valid driver’s license and follow safe driving techniques

· Possess regular access to a working vehicle

· Provide certificate of automobile insurance per policy

Physical Abilities

· Ability to read, write and communicate effectively in English

· Bilingual Spanish preferred

· Ability to lift and/or move at least 10 pounds

· Ability to visit clients in their homes.

· Visual acuity to read a computer screen and other documents

· Ability to utilize the telephone and respond to callers

Additional Duties

· Understand the social/emotional component and actively participate and apply reflective practices both internally and externally

· Verify/re-verify client eligibility for services initially and annually

· Develop an appropriate Individualized Family Service Plan (IFSP) within 45 days of referral

· Actively facilitate each IFSP meeting, following EI protocols and ensuring compliance with EI Principles, financial and insurance requirements, eligibility, and plan development

· Function as client advocate while empowering families to assume that role

· Coordinate referral process within or outside agency, in line with individual’s needs. Assure choice of provider as allowed under the Early Intervention System

· Facilitate timely referrals to Community Access Coordination program and Autism Support Program, along with ongoing collaboration and information sharing as needed for eligible children and as allowed by the family’s consent

· Ensure that each child is provided services in the natural environment, unless extreme circumstances warrant services in the center

· Facilitate timely transitions to the local school district for each child turning three in order to have special education eligibility determined prior to the child’s third birthday

· Complete an exit review of every eligible child leaving Early Intervention within 120 days prior to file closure

· Complete a transition meeting for every eligible child at least 90 days prior to the child’s third birthday

· Facilitate transition of the child to any other services prior to the child’s third birthday

· Participate in Child and Family Connections’ playgroup as needed or assigned

· Establish and maintain cooperative working relationships with all McHenry County Behavioral Healthcare Network Agencies, specialized service providing agencies, and local school districts

· Participate in agency informational, planning and management meetings

· Participate in local or statewide task forces as assigned by the program manager

· Attend in-service events to maintain and enhance skills

· Actively support agency events through activities prior, during and after event. Participation in at least one agency event each year is required.
