About Us

We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).

Job Summary

As a Hospice RN Case Manager, you'll play a crucial role in planning, organizing, and directing hospice care. This position requires a blend of nursing expertise, community health education, and a deep commitment to end-of-life care. You'll be the cornerstone in building a bridge between our organization's resources and the needs of patients and families in their homes and communities.

Key Benefits

Salary: $75,000 - $85,000 per year

Core Responsibilities

Conduct patient and family assessments
Develop and implement personalized care plans
Provide skilled nursing care and pain management
Coordinate with interdisciplinary team members
Maintain accurate patient records
Participate in on-call duties

Qualifications

We're excited to train RN's that are passionate about hospice care
RN license
1-2 years recent acute care experience (preferred)
1-2 years home care experience (preferred)
Valid driver's license and reliable vehicle
Strong communication and interpersonal skills
Empathy and comfort with end-of-life care

Work Environment

Mix of facility and home-based care
Average caseload: 12-15 patients
Monday – Friday, 8am-5pm (some on-call and weekends)
Centralized territory assignment
