Full Job Description

Job Overview

Keystone Post Acute Care is seeking an experienced, compassionate, and motivated Social Services Director (SSD) to lead our Social Services Department. This position plays a critical role in advocating for residents, coordinating discharge planning, leading care conferences, and ensuring compliance with all federal, state, and facility regulations.

Position is Full Time Monday-Friday, sometimes weekends. Pay is based on experience

Duties

Coordinate and oversee all Social Services functions within the facility.
Complete comprehensive psychosocial assessments and develop individualized care plans.
Lead and actively participate in interdisciplinary care conferences.
Identify barriers to discharge upon admission and develop timely discharge plans.
Coordinate safe and effective resident discharges with families, physicians, hospitals, home health agencies, and community resources.
Maintain accurate, complete, and timely documentation in the electronic medical record.
Assist residents and families with adjustment, counseling, financial resources, advance directives, and community referrals.
Ensure resident rights are protected and concerns are addressed promptly.
Investigate and resolve resident and family grievances.
Coordinate admissions, transfers, resident-initiated discharges, and other required notifications in accordance with facility policy and regulatory requirements.
Collaborate closely with Nursing, Therapy, Dietary, Activities, Admissions, Case Management, and Administration.
Maintain compliance with CMS, California Department of Public Health (CDPH), and facility policies and procedures.
Participate in Quality Assurance Performance Improvement (QAPI) initiatives and regulatory surveys.

Qualifications:

Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, or related field preferred.
Experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting.
Knowledge of Medicare, Medi-Cal, managed care, discharge planning, and post-acute services.
Excellent communication and conflict-resolution skills.
Strong documentation and organizational skills.
Ability to work independently while collaborating with an interdisciplinary team.

Preferred:

Previous experience as a Social Services Director.
Knowledge of PointClickCare (PCC).
Experience with California SNF regulations and CDPH surveys.
