Full Job Description

Job Summary
We are seeking a compassionate and highly skilled Home Health Nurse to provide comprehensive patient care in a home and community setting. The ideal candidate will possess extensive clinical expertise, including experience with medical documentation, EMR/EHR systems, and specialized procedures such as IV infusion, catheter care, and wound management. This role involves assessing patient needs, developing care plans, and delivering personalized nursing interventions to promote health and recovery. The Home Health Nurse will work closely with patients of all ages, including those with complex medical conditions such as geriatrics, pediatrics, developmental disabilities, and post-acute care needs. A strong understanding of HIPAA regulations, infection control protocols, and medical terminology is essential for success in this position.

Responsibilities

Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations.
Administer medications, injections, IV infusions, and perform venipuncture with aseptic technique.
Manage complex wound care including suturing, ostomy care, and dressing changes.
Provide specialized treatments such as botulinum toxin injections and esthetic laser treatments where applicable.
Assist with tube feeding, ventilator management, and airway maintenance for patients requiring advanced respiratory support.
Coordinate discharge planning and collaborate with interdisciplinary teams to ensure continuity of care across settings such as hospitals, nursing homes, assisted living facilities, and outpatient clinics.
Document all patient interactions accurately using EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
Educate patients and families on disease management, medication administration, nutrition, and health coaching tailored to individual needs.
Ensure compliance with HIPAA standards and infection control protocols during all patient interactions.
Participate in case management activities related to trauma medicine, infectious disease care, behavioral health issues, palliative medicine, or specialty areas such as cardiology or nephrology as needed.

Requirements

Valid RN license with current state licensure; additional certifications such as BLS/CPR are required.
Proven experience in diverse clinical settings including hospital medicine (ICU/CCU/Trauma), outpatient clinics, home care environments or long-term care facilities.
Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD-9/10 coding and CPT coding is preferred.
Strong knowledge of medical terminology, anatomy & physiology, diagnostic evaluation techniques (sonography), and medication administration protocols.
Experience with specialized procedures including IV insertion/infusion therapy, catheterization (urinary or central lines), ostomy management, and sterile processing techniques.
Ability to work independently in a home setting while managing multiple patients efficiently.
Excellent communication skills for working with patients of all ages—including children with developmental disabilities or neonatal intensive care patients—and collaborating with healthcare teams.
