Full Job Description

B&D Integrated Health Services is seeking a highly motivated and clinically skilled Qualified Professional (QP) to join our Intensive In-Home (IIH) division. This role is ideal for an experienced child and adolescent mental health professional who thrives in community-based work and is committed to delivering high-quality, family-centered behavioral health services.

The QP plays a critical role in coordinating care, leading person-centered planning, and collaborating with families, community partners, and multidisciplinary providers to support youth with complex behavioral health needs. This position reports directly to the IIH Leader and indirectly under IIH Management Lead and requires in-home and community-based service delivery.

Key Responsibilities

Clinical Coordination & Treatment Planning

Lead initial and ongoing comprehensive assessments for youth and families enrolled in IIH services
Convene and facilitate Child and Family Team (CFT) meetings using a person-centered approach
Develop, implement, and routinely update Person-Centered Plans (PCPs)
Monitor clinical progress, evaluate intervention effectiveness, and ensure measurable outcomes

Collaboration & Care Integration

Coordinate with medical and behavioral health providers (e.g., primary care, psychiatry)
Collaborate with community partners including schools, DSS, DJJDP, and natural supports
Ensure timely referrals and linkage to appropriate psychological and psychiatric evaluations

Service Delivery & Documentation

Provide and coordinate behavioral health interventions in collaboration with licensed clinicians and CFT members
Deliver psychoeducation and support to youth and caregivers as outlined in the PCP
Maintain accurate, timely, and compliant clinical documentation in accordance with state regulations

Crisis Support & Quality Assurance

Assist with crisis intervention and stabilization services as needed
Support the Team Leader in monitoring treatment outcomes and service quality
Complete all required state-mandated trainings and maintain compliance with NC Administrative Code

Required Qualifications

Must meet Qualified Professional (QP) requirements per North Carolina Administrative Code
At least a bachelor degree
Minimum of 2 years of experience providing mental health and/or substance abuse services to children and adolescents
Prior experience in Intensive In-Home Services (IIH) strongly preferred
Strong clinical judgment, case management, organization, and documentation skills
Excellent written and verbal communication skills
Ability to work effectively in home-based and community settings
