Organization Description

Cornerstone Community Outreach (CCO) is a well‑established Chicago nonprofit—founded in 1989—with a powerful mission: Addressing Homelessness, Providing Shelter, Accepting People, Finding Home.

Located in Uptown, Chicago, our programs provide shelter for single women, single men, intact families, and single parent/guardian families. Our wraparound services include casework, three meals a day, a food pantry, and a “free” store with gently used clothes, baby items, and household items.

They operate four core programs across multiple properties:

Hannah Shelter, for single mothers and children
Sylvia Shelter, for intact families of all kinds
Naomi Shelter for Women
Naomi Shelter for Men

Job Summary:

The Case Manager will provide direct support to individuals and families experiencing homelessness and housing instability. This position plays a vital role in helping clients navigate barriers, secure stable housing, and achieve longterm self-sufficiency through a trauma-informed, strengths-based approach.

Key Responsibilities

Conduct intake assessments and develop individualized service plans tailored to client needs and goals.
Provide ongoing case management, including crisis intervention, referrals, advocacy, and follow-up support.
Assist clients in obtaining vital documents, benefits, employment, housing, and other essential services.
Maintain regular contact with clients to monitor progress, revise goals, and offer continued encouragement.
Collaborate with internal teams, community partners, landlords, and social service agencies to coordinate care.
Ensure accurate and timely documentation in client files and databases (e.g., HMIS or other data systems).
Participate in staff meetings, case conferences, and professional development training.
Uphold CCO’s mission, policies, and procedures while promoting a respectful and inclusive environment.

Qualifications:

Bachelor’s degree in Social Work, Psychology, Human Services, or related field (or equivalent work experience).
Minimum one year of experience in case management, housing services, or related social service work.
Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse populations.
Familiarity with issues related to homelessness, mental health, substance use, and trauma.
Ability to work independently and as part of a collaborative team.
Proficient with Microsoft Office and client databases (e.g., HMIS preferred).
Bilingual a plus

Other Requirements

Must pass a background check and sex offender screening
Possible weekends and evenings if needed
