Location: Jamestown, NY
Schedule: Full-Time
Status: Provisional Appointment (Civil Service)

Help Individuals Build a Brighter Future

The Chautauqua County Department of Mental Hygiene & Social Services is seeking a compassionate, motivated, and bilingual professional to join our team as a Case Manager (Spanish Speaking) in our Jamestown office.

In this role, you will work directly with English- and Spanish-speaking individuals and families to help them overcome barriers to employment, achieve self-sufficiency, and access community resources. You'll provide guidance, advocacy, case management, and supportive services that empower clients to reach their personal and employment goals.

As a Case Manager, you will:

Meet with clients to assess strengths, barriers to employment, and supportive service needs.
Develop individualized employability and self-sufficiency plans and update them according to program requirements.
Coach clients on employment readiness, job search strategies, interviewing skills, workplace expectations, and financial management.
Connect participants with community resources including education, job training, childcare, transportation, housing, financial aid, and other supportive services.
Coordinate and monitor all aspects of participant services while evaluating progress toward self-sufficiency goals.
Explain County Health and Human Services programs, eligibility requirements, and regulations.
Maintain detailed case records, case notes, reports, and required documentation.
Conduct office, home, worksite, and community visits to monitor participant progress and provide ongoing support.
Facilitate workshops, orientation sessions, support groups, and other educational activities.
Advocate for clients with employers, educational institutions, and community organizations.
Work collaboratively with Transitional Assistance staff and community partners to coordinate services.
Assist clients during emergency situations and provide crisis support when appropriate.
Serve as a domestic violence liaison by connecting clients with appropriate supportive services.
Assist with transportation to interviews, apartment searches, childcare arrangements, and other activities that support client success.
Remain available to clients through office appointments and telephone contact.

The ideal candidate is:

Fluent in both English and Spanish with strong verbal and written communication skills.
Passionate about helping individuals achieve independence and employment success.
An effective communicator with excellent interpersonal skills.
Organized and able to manage multiple cases simultaneously.
Comfortable working independently while collaborating with community agencies and coworkers.
Skilled at problem-solving, advocacy, and motivating clients toward their goals.

Minimum Qualifications

Applicants must possess:

Successful completion of 60 semester credit hours from a regionally accredited or New York State registered college or university; and
Two (2) years of experience involving direct client contact in the delivery of services within a human services agency or program.

Additional Requirements

Ability to meet the transportation requirements necessary to perform field visits throughout Chautauqua County.
Must satisfy New York State driving requirements for this position, including no alcohol- or drug-related driving convictions or refusals to submit to a Blood Alcohol Content (BAC) or chemical test within the three years preceding examination, certification, and appointment.
Bilingual Requirement: Candidates must demonstrate the ability to communicate effectively in both English and Spanish. Language proficiency will be evaluated by the appointing authority and/or supervisor during the probationary period.

Civil Service Information

This position is being filled on a provisional basis pending the administration of a future Chautauqua County Civil Service examination. Continued employment will be contingent upon successfully meeting Civil Service requirements.

Pay: $24.42 - $31.51 per hour
