Job Overview

We are seeking a dynamic and dedicated Registered Nurse Assessment Coordinator (RNAC) to join our healthcare team. In this vital role, you will lead comprehensive patient assessments, coordinate care plans, and ensure compliance with all relevant healthcare regulations. Your expertise will help optimize patient outcomes, streamline clinical workflows, and uphold the highest standards of nursing practice. This position offers an exciting opportunity to influence patient care quality while demonstrating strong leadership and clinical skills in a fast-paced environment.

Duties

Conduct thorough assessments of patients’ health status, including physiological and medical history evaluations, to develop personalized care plans.
Coordinate multidisciplinary team efforts to ensure seamless patient care delivery across hospital or long-term care settings.
Manage and oversee medical documentation, ensuring accuracy and adherence to HIPAA (Health Insurance Portability and Accountability Act) regulations for patient privacy.
Utilize Electronic Health Records (EHR) systems to document assessments, treatment plans, and progress notes efficiently.
Analyze clinical data to identify trends, support quality improvement initiatives, and ensure compliance with state healthcare regulations and long-term care standards.
Provide clinical supervision and guidance to nursing staff and administrative personnel involved in patient assessments.
Collaborate with management to develop policies that enhance patient care quality while maintaining regulatory compliance.

Requirements

Valid nursing license with current registration in the applicable state.
Extensive hospital or long-term care facility experience, including acute care settings.
Strong knowledge of physiology, anatomy, medical terminology, and healthcare regulations such as HIPAA and state-specific long-term care laws.
Proficiency in Microsoft Office applications and EMR/EHR management systems for medical documentation and data analysis.
Demonstrated management skills with experience supervising nursing teams or administrative clinical staff.
Familiarity with medical records management, medical documentation standards, and electronic health records (EHR) systems.
Excellent communication skills for effective collaboration with multidisciplinary teams and patients.
Ability to interpret complex medical data accurately while ensuring regulatory compliance.
Knowledge of healthcare quality improvement processes and long-term care regulations is highly desirable.
