Full Job Description

ABOUT THE ORGANIZATION

The Center for Disability Rights is a Disability-led organization where a majority of the board members, management staff, and staff are themselves people with disabilities. This critical difference sets the organization apart from other organizations that were established to serve and support people with disabilities but are not governed, managed and staff by people with disabilities themselves.

Job Summary:

The Transition Team Leader is a front-line supervisory position responsible for overseeing the work of Transition Specialists (TS) who assist individuals with disabilities seeking community living as an alternative to institutionalization in a nursing facility. The Transition Team Leader supports the Transition Specialists in a hands-on way and is responsible for motivating employees, managing their performance, providing them with guidance and feedback, and resolving conflicts. The Transition Team Leader is the direct point of contact between senior management and their direct reports. In addition to the supervisory responsibilities, the Transition Team Leader provides direct services.

Job Duties:

Hire, train, supervise, and manage the Transition Specialist staff.
Provide and document one-on-one supervision with all direct reports, at least on a biweekly basis and more frequently as needed.
Assign staff caseloads and review case notes on a weekly basis to ensure staff provide high quality services and effectively document those services, providing feedback as needed.
Produce and review weekly reports for documenting, monitoring, and assessing staff productivity.
Provide transition services for individuals with disabilities seeking a community-based alternative to institutionalization.
Ensure completion of project and contract deliverables.
Coaching and training staff to improve performance as necessary.
Provide direct support to line staff by problem solving participant issues and advocacy concerns.
Lead weekly staff meetings and support monthly departmental meetings working with the Housing Team Leader and the Director of Housing/Transition.
Provide the core Transition services to individual participants.
Engage in and document community outreach activities, and provide and document technical assistance to the community.
Report any issues and challenges to the Director of Housing and Transition.
Provide information for reports as requested.
Other duties as assigned.

Qualifications:

Bachelor’s degree in human service or related field and at least five years of experience.
Experience with case management, service coordination or care management is desired.
Personal and lived experience with disability preferred.

Skills & Competencies:

Familiarity with various community-based services, HCBS Waiver and MLTC programs.
Comfort in working with people with physical, mental and intellectual disabilities.
Demonstrated commitment to self-direction, person-centered planning and consumer choice.
Strong personal and written communication skills.
Excellent interpersonal, negotiation, and conflict resolution skills.
Strong organizational skills and attention to detail.
Impeccable time management skills with a proven ability to meet deadlines.

This is an in-office based position. Normal business hours are Monday - Friday 9:00 am to 5:00 pm.

Can you share an example of how you used short-term counseling or advocacy to help a family feel confident about their loved one leaving a facility?
