Social Services Director (SSD) – Skilled Nursing Facility

Join Our Team

Are you a compassionate and experienced Social Services professional who is passionate about supporting residents and families in a skilled nursing environment? We are looking for a dedicated Social Services Director (SSD) to lead our Social Services Department and help ensure residents receive the emotional, social, and psychosocial support they need to thrive.

In this role, you will work closely with residents, families, facility leadership, and the interdisciplinary care team to promote quality of life, protect resident rights, support discharge planning, and maintain compliance with applicable regulations.

What You’ll Do

· Plan, organize, develop, implement, evaluate, and direct the overall operation of the Social Services Department.

· Participate in care plan meetings, resident and family meetings, interdisciplinary team discussions, discharge planning meetings, and quality assurance activities.

· Support residents and families during admission, adjustment to facility life, change of condition, discharge, transfer, or end-of-life transitions.

· Coordinate discharge planning, including referrals to community resources, home health, durable medical equipment, hospice, behavioral health, ombudsman services, and other external agencies as appropriate.

· Promote and protect resident rights, dignity, self-determination, privacy, and quality of life.

· Respond professionally to resident, family, and staff concerns, grievances, and psychosocial needs, including crisis intervention and conflict resolution.

· Collaborate with nursing, therapy, admissions, activities, business office, administration, and outside providers to support resident-centered care.

· Maintain compliance with applicable regulations, documentation standards, facility policies, and survey readiness expectations.

· Develop and maintain positive working relationships with residents, families, staff, physicians, hospitals, case managers, community partners, and regulatory representatives.

What We’re Looking For

· Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Gerontology, or a related field required; Master’s degree preferred.

· Minimum of two years of experience in a skilled nursing, long-term care, healthcare, hospital, or social services setting preferred.

· Prior experience as a Social Services Director, Social Services Designee, or similar leadership role preferred.

· Knowledge of resident rights, discharge planning, care planning, psychosocial assessments, and long-term care regulations preferred.

· Current social work license or certification preferred, if applicable based on state requirements.

· Ability to read, write, speak, and understand English clearly; bilingual skills are a plus.
