Full Job Description

Job Summary
Plans, organizes and coordinates Family Self-Sufficiency programs and services with agencies and groups. Aids and promotes welfare of Section 8 participants, and provides information concerning regulations, facilities, and services by performing the following duties. Submits complete, accurate reports in a timely manner.

Responsibilities

Act as the primary point of contact for residents, addressing their needs and concerns with enthusiasm and professionalism
Coordinate access to community resources such as childcare, behavioral health services, Section 8 housing assistance, and workforce development programs
Provide crisis intervention and crisis management support to residents experiencing emergencies or urgent issues
Offer addiction counseling, behavioral health guidance, and motivational interviewing to promote positive change
Supervise and mentor residents, including children, fostering a supportive environment that encourages growth and development
Assist residents with navigating complex systems such as Medicare, child welfare services, or housing programs
Organize community events, workshops, and activities that promote social engagement and resident well-being

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Develops and implements strategies to assist Program participants in becoming self-sufficient

· Serves as liaison between Program participants and the community

· Secures social services such as health, welfare, and education programs for improving family and community standards

· Provides leadership to FSS participants in development of group activities such as adult education and recreation

· Accurately enters client data into computer system

· Refers families with personal problems to appropriate community resources

· Manages caseload of participants for individuals enrolled in FSS Program

· Compiles and submits to Supervisor monthly report on FSS participants, including program goals, and participation of program. Achieves enrollment and escrow participation goals as established in the HASB Annual Plan.

· Graduate 90% of the FSS enrollees as established by the term of their plan

· Maintain FSS files in audit-ready condition with no exceptions

· Provides counseling services to assist FSS participants in overcoming the barriers to self-sufficiency

· Facilitates individual and group conferences to analyze problems and determine appropriate course of action

· Refers FSS participants to supportive services, such as childcare, remedial, secondary, and post-secondary education, job training, counseling, and placement, substance abuse counseling and limited treatment referrals.

· Complies records and prepares reports.

· Reviews service plans and performs follow-up to determine quantity of service provided to FSS participants and determines status of client’s case

· Represent HASB in the community
