Job Overview
We are seeking a dedicated and energetic Wound Nurse to join our dynamic healthcare team. In this vital role, you will provide expert wound care management, utilizing advanced techniques and evidence-based practices to promote healing and improve patient outcomes. Your expertise will support a diverse patient population across various settings, including acute care, long-term care, and community health environments. This position offers an exciting opportunity to make a meaningful difference in patients' lives through compassionate care, innovative treatment modalities, and collaborative teamwork.

Responsibilities

Assess patients’ wound conditions through comprehensive physical examinations and detailed patient histories, ensuring accurate diagnosis and treatment planning.
Implement advanced wound care protocols, including the use of dressings, debridement techniques, and sterile procedures following aseptic technique standards.
Consult with the Care Planning Team concerning assessment evaluations and assist in planning and developing the skin care treatment to be performed for the resident.
Collaborate with multidisciplinary teams to develop individualized care plans that incorporate pain management, infection control, and nutritional support to optimize healing.
Educate patients and their families on wound management techniques, prevention strategies, and post-treatment care to promote self-care and reduce recurrence.
Document all patient interactions meticulously within electronic health record (EHR) systems like Ponit Click Care (PCC), adhering to HIPAA regulations and ensuring accurate medical records.
Ensures that nursing department employment actions are administered fairly and in compliance with the facility’s Policy Against Discrimination, Harassment and Retaliation.

Experience

Valid Nursing license with current state licensure required; additional certifications in wound care (e.g., Wound Care Certified - WCC) preferred.
Prefer a minimum, 1 year of experience as a licensed Nurse in a skilled nursing facility, or other related health care facility.
Familiarity with EMR/EHR systems like Point Click Care for accurate documentation of patient assessments and treatment plans.
Background working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia care needs.
Knowledge of infectious disease protocols, pain management strategies, and infection control practices essential for safe wound treatment.
Experience in specialized areas such as hospice & palliative medicine, occupational health clinics, or post-acute care settings is highly valued.
