Overview
Join our dynamic healthcare team as a Registered Nurse Case Manager, where your expertise will drive exceptional patient outcomes through comprehensive case management and coordinated care. In this vital role, you will leverage your critical care, hospital experience, and specialized knowledge to assess patient needs, develop personalized care plans, and facilitate seamless transitions across inpatient, outpatient, and post-acute settings. Your proactive approach and dedication will ensure patients receive the highest quality of care while optimizing resource utilization and discharge planning. Duties

Conduct thorough patient assessments including vital signs, physiology evaluation, and medical records review to determine care needs.
Coordinate multidisciplinary care plans for diverse populations such as pediatrics, geriatrics, behavioral health, and trauma patients in outpatient settings.
Manage utilization review and medical-surgical case management to ensure appropriate level of care and resource allocation.
Facilitate discharge planning by collaborating with physicians, social workers, and community providers to arrange post-acute care services.
Oversee diagnostic evaluations and medical records documentation to support clinical decision-making and compliance with HIPAA regulations.
Perform intake assessments for outpatient services, ensuring smooth patient onboarding and service delivery.
Utilize clinical expertise to guide patient care strategies effectively.

Qualifications

Valid nursing license with current registration in the relevant state or jurisdiction.
Strong background in hospital medicine, medical-surgical units, post-acute care settings, outpatient clinics or managed care environments.
Demonstrated knowledge of physiology, medical terminology, patient assessment techniques, and discharge planning processes.
Experience with utilization management, diagnostic evaluation coordination, and case management procedures.
Familiarity with HIPAA compliance standards and medical records documentation practices.
