Full Job Description

MemoryCare is a dedicated non-profit organization committed to providing compassionate, dementia care for older adults with cognitive disorders that also emphasizes caregiver education, counseling, and support. Since 2000, MemoryCare has been empowering families and enhancing quality of life for individuals facing memory disorders through a multidisciplinary team approach that integrates medical management for people with cognitive disorders with caregiver support services.
MemoryCare is seeking an experienced Care Manager to join our team. Your leadership will help families navigate complex behavioral health challenges, manage crises effectively, and access essential community resources. This position offers an opportunity to make a meaningful difference in the lives of those affected by memory disorders while working within a collaborative and supportive environment.

Some job highlights include (list may be incomplete)

Demonstrate professionalism to anyone interfacing with our programs as a representative of MemoryCare

Input relative information and data /cognitive testing of patient/vital signs/family history into the EMR

Assist physician in physical exam and necessary procedures as appropriate

Schedule follow-up visits

Respond to all patient/family related queries via phone, email, or electronic medical record messaging and handling triaging, as appropriate, to physician staff; assist families with resource needs based on direction of physician

Ensure documentation of care management interactions for physician review and direction.

Manage the team's phone triage and scheduling when scheduler not available

Provide resource assistance to caregivers for educational purposes (literature/info searches; community resources)

Organize and maintain patient/family education files

Assist with resource management in the Eldercare Resource Center as requested

Support the monthly requirements of the Medicare GUIDE program for families enrolled in this program

Qualifications

Proven education and experience in social work, behavioral health, or related human service fields
Excellent written communication and technology skills
Compassionate approach combined with strong organizational skills to manage multiple appointments and caregiver contacts effectively each day
Ability to handle emotional situations calmly while maintaining a positive and professional outlook
