Full Job Description

Job Title: Hospice Registered Nurse (RN)
Location: Nashville, TN 37209
Duration: 13 weeks
Shift: Day 5x8-Hour (08 am-05 pm)

Job Description:

We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide high-quality, patient-centred hospice care to terminally ill patients in their homes and other care settings. The RN will perform comprehensive patient assessments, develop and implement individualised care plans, coordinate with an interdisciplinary hospice team, and provide emotional, educational, and clinical support to patients and their families. This role is responsible for managing assigned patient caseloads, maintaining accurate clinical documentation, ensuring compliance with hospice regulations, and promoting comfort, dignity, and quality of life throughout the end-of-life care journey.

Key Responsibilities:

Provide skilled nursing care to hospice patients according to physician orders and individualized care plans.
Perform initial and ongoing patient assessments to evaluate physical, emotional, psychosocial, and spiritual needs.
Develop, implement, and update the Hospice Plan of Care in collaboration with the interdisciplinary team.
Coordinate care with physicians, Licensed Practical/Vocational Nurses (LPN/LVNs), Certified Nursing Assistants (CNAs), social workers, chaplains, and other healthcare professionals.
Educate patients, families, and caregivers on disease progression, symptom management, medication administration, oxygen therapy, wound care, and hospice philosophy.
Monitor and report changes in patient condition to the attending physician and hospice physician.
Maintain accurate, timely, and compliant clinical documentation, including assessments, care plans, progress notes, and interdisciplinary records.
Supervise hospice aides and conduct required monthly supervisory visits to evaluate care quality and competency.
Ensure adherence to infection control practices, safety standards, and regulatory requirements.
Participate in Quality Assessment and Performance Improvement (QAPI) initiatives and staff meetings.
Assist with scheduling coordination and may participate in the agency's on-call rotation.
Provide compassionate emotional support and bereavement resources to patients and their families.

Requirements:

Active Registered Nurse (RN) license in the state of Tennessee.
Minimum 1 year of RN experience in acute care, oncology, long-term care, hospice, or home health (clinical internships may be considered in some cases).
Current BLS Certification (American Heart Association).
Valid driver's license, reliable transportation, and current automobile insurance.
Strong patient assessment, case management, documentation, and communication skills.
Knowledge of hospice philosophy, end-of-life care, Medicare/Medicaid regulations, and interdisciplinary care coordination.
Ability to use electronic documentation systems, computers, and standard office equipment.
