Full Job Description

The Enhanced Care Management (ECM) Outreach Coordinator is responsible for screening, outreach, and assisting with enrollment of potential ECM members into Centric Health’s ECM Program. Outreach efforts include telephonic and in-person outreach in the community, clinic, or office. The staff is responsible for keeping detailed records of their efforts and communicating regularly with their direct supervisor. The position is responsible for reviewing and screening qualified members who are referred to the ECM Program. The staff fosters and maintains relationships with community partners and Centric Health providers to facilitate the referrals and enrollment of potential ECM members. This position requires strong interpersonal skills to build rapport with members and work closely with the ECM multidisciplinary care team (MDT) supporting ECM members.

RESPONSIBILITIES

· Conduct outreach and enrollment activities, including performing both in-person and on the phone outreach to eligible and existing members to promote program enrollment and/ or retention.

· Educate members on ECM Program benefits and services to promote program enrollment.

· Perform eligibility and medical coverage screenings.

· Document all outreach activities and member encounters in the Case Management System and collect required information to complete the enrollment process, including completion of screening forms and questionnaires.

· Participate in community outreach events and member engagement activities.

· Maintain and handle confidential member information.

· Demonstrate strong verbal communication skills to leave a positive impression on every member and/or provider encounter

· Provide follow-up on all pending applications to ensure the enrollment process is complete.

· Provide information in a manner that is culturally and linguistically appropriate for members.

· Maintain communication with supervisor and core team members as well as any other pertinent staff to meet program requirements and meet member needs.

· Maintain statistics/data on contacts; identify needs, problems, and service gaps.

· Maintain a collaborative relationship with local community agencies, such as shelters, local schools, churches, county agencies, and key stakeholders to facilitate referrals to the ECM Program.

· Assist the ECM team to understand the culture, norms, beliefs and preferences of the members and their community by representing the voice of the community.

· Ensures documentation is accurate, useful and in compliance with regulatory requirements and accreditation standards.

· Participates in all appropriate staff meetings or other activities as needed.

· Perform other duties as assigned that support organizational goals and objectives.

EDUCATION & EXPERIENCE

· High School diploma or GED required.

· Minimum of 2 years of experience working in a healthcare or community health setting.

· Experience in a managed health care environment preferred (IPA, HMO, or Health Plan)

· Bilingual proficiency in English and Spanish is preferred

KNOWLEDGE & SKILLS

· Willingness to collaborate as part of a team with professionals at all levels to achieve goals and remove barriers to member health.

· Sensitivity to members' social, cultural, language, physical, and financial differences.

· Ability to prioritize tasks based on situational needs.

· Ability to work independently, organize, and prioritize multiple tasks throughout the day.

· Strong attention to detail and ability to be accurate, thorough, and persistent in problem-solving and task completion.

· Excellent verbal and written communication skills with the ability to communicate effectively with all levels of the organization and members.

· Proficiency in creating professional documents with proper grammar and punctuation.

· Ability to maintain professionalism and adapt to a changing environment.

· Ability to understand and communicate complex health and benefit information.

· Proficient in the use of common office technology, including electronic Case Management systems.

· General knowledge of Microsoft Office applications such as Excel, Word, and Outlook.

· Effectively use general office equipment, including desktop computers, copy machines, scanners, facsimile machines, and telephone equipment.

· Reliable in attendance and adherence to work schedule and business dress code.

· Ability to always maintain strict confidentiality.
