Full Job Description

JOB TITLE: Intake Specialist

FLSA STATUS: Non-Exempt

PAY: The hourly range for this position is $23 - $25/hour

FT/PT: Full-time (35 hours/week)

If so, North Shore Senior Center could be the perfect place for you! We are currently looking for a dedicated Intake Specialist to join our team.

Our wide range of services includes specialized dementia care, social services, educational and wellness programs, and volunteer opportunities, all designed to enrich the lives of older adults across the 26 communities we serve.

SUMMARY: The Intake Specialist is responsible for the initial screening of Senior and Family Services clients, providing information and referrals to callers, assisting with distribution of screens for the Choices for Care program and a variety of administrative tasks.

DUTIES AND RESPONSIBILITIES:

Gathers and accurately documents relevant information from potential clients in order to make first level assessments of need and eligibility. May provide information and referral sources. Collects the information required to complete next level assessments. Asks all the questions necessary to complete the National Aging Program Information Systems (NAPIS) form which is the foundation for the department’s client and case records.
Responds to phone and walk-in requests by providing information on programs or services and facilitates referral to services if needed. Classifies initial contact for potential client or referral source by completing a National Aging Program Information Systems (NAPIS) form, providing information and assistance and sending agency and program brochures as appropriate.
Utilizes both internal and external resources to provide information for clients and Care Coordinators. Assigns clients to Care Coordinators using the Care Coordinator assignment rotation list. Completes Adult Protective Services (APS) Intake and forwards the reports to the APS Supervisor on the same day the report was taken.
Maintains updated forms for all Senior & Family Services Department programs and ensures availability of needed forms. Collaborates with other Intake Specialists to complete reports requested by Senior & Family Services Department supervisors.
Records minutes at Senior & Family Services Department staff meetings.

QUALIFICATIONS:

Bachelor's Degree (BA) from four-year college or university preferred, or one to two years of related experience and/or training, or equivalent combination of education and experience
At least one year of experience working with older adults
Certificates, licenses and registrations preferred: Inform USA certification
Computer skills required: Intermediate knowledge of standard computer programs including Microsoft Office Suite of programs
Other skills required: Sensitivity to cultural diversity
