Full Job Description

Position Summary

Always Home Care is seeking an experienced and highly organized Infusion Services Manager / Infusion Specialist to oversee and grow our home infusion services throughout New York.

The Infusion Services Manager will be responsible for coordinating infusion cases from referral through start of care, communicating with patients, physicians, pharmacies, hospitals, insurance plans, and field nurses, and ensuring that all infusion services are delivered safely, efficiently, and in accordance with company policies and applicable New York State requirements.

The ideal candidate has strong home infusion experience, excellent communication and organizational skills, and the ability to manage both the clinical and operational aspects of an infusion program.

Responsibilities

Manage the day-to-day operations of the Always Home Care infusion program.
Receive and review new infusion referrals and determine whether the agency can safely accept the case.
Coordinate start-of-care dates and nursing schedules.
Communicate with physicians, hospitals, pharmacies, insurance companies, patients, and family members.
Verify that appropriate physician orders and infusion documentation are received before services begin.
Coordinate delivery of medications, supplies, pumps, and other necessary infusion equipment.
Recruit, coordinate, and manage qualified infusion nurses.
Ensure nurses are appropriately trained and competent for assigned infusion cases.
Match nurses to patients based on location, availability, experience, and clinical requirements.
Monitor active infusion cases and immediately address scheduling or clinical-service concerns.
Track hospitalizations, missed visits, changes in patient condition, medication changes, and discontinuation of therapy.
Ensure timely and complete nursing documentation.
Review clinical documentation for accuracy, completeness, and compliance.
Maintain communication with patients and families throughout the course of treatment.
Coordinate PICC line, central-line, peripheral IV, port, and other infusion-related cases as appropriate.
Assist with IV antibiotics, hydration therapy, biologic medications, and other physician-ordered infusion therapies within the agency's authorized scope of services.
Maintain infection-control and patient-safety standards.
Investigate complaints, incidents, missed visits, and service issues.
Work closely with Administration, Nursing, Intake, HR, Scheduling, Billing, and Compliance departments.
Monitor authorizations, visit utilization, staffing costs, and reimbursement to help ensure cases are financially viable.
Develop relationships with hospitals, pharmacies, physicians, discharge planners, and referral sources.
Assist with growing the infusion division and developing new referral relationships.
Maintain accurate reports regarding referrals, active patients, completed cases, staffing, revenue, and operational performance.
Participate in quality assurance, compliance reviews, staff education, and performance improvement activities.
Perform other duties related to the operation and growth of the infusion program.

Qualifications

Prior home infusion, specialty pharmacy, home healthcare, or infusion nursing experience required.
Strong knowledge of infusion-service coordination and patient care.
Experience working with pharmacies, hospitals, physicians, insurance companies, and field nurses.
Strong understanding of IV therapy documentation and safety requirements.
Excellent organizational and case-management skills.
Ability to manage multiple patients and referrals simultaneously.
Strong written and verbal communication skills.
Strong leadership and problem-solving abilities.
Ability to work independently and take ownership of the infusion department.
Comfortable working in a fast-paced home healthcare environment.
Computer proficiency and experience with healthcare documentation or scheduling systems.

Clinical Qualification

For a position responsible for clinical supervision, patient assessment, nursing direction, or other functions requiring professional nursing judgment:

Active and unrestricted New York State Registered Professional Nurse (RN) license required.

Home infusion, IV therapy, PICC/central-line, or specialty infusion experience is strongly preferred.

Preferred Experience

2+ years of infusion or home infusion experience.
Previous management or supervisory experience.
Home healthcare agency experience.
Specialty pharmacy experience.
Experience developing or growing an infusion program.
Knowledge of New York home care regulations and compliance requirements.
Experience with Medicare, Medicaid, managed care, and commercial insurance authorizations.
