Full Job Description

Job Summary

Silver Lantern Homecare is seeking an organized, compassionate, and highly motivated Care Team Coordinator to oversee the day-to-day operations of our growing home care agency. This position is essential in ensuring exceptional client care through effective caregiver staffing, client coordination, compliance, and administrative support.

The ideal candidate thrives in a fast-paced environment, is an excellent problem solver, and has hands-on caregiving experience that allows them to confidently support both clients and caregivers.

Essential Responsibilities

(Monday-Friday, 9am-5pm; Rotational on-call weekend)

Staffing & Scheduling

Coordinate daily caregiver schedules based on client needs, caregiver skills, availability, and geographic location.
Manage schedule changes, call-outs, and last-minute staffing needs to ensure uninterrupted client care.
Communicate effectively with caregivers, clients, and families regarding scheduling updates.
Maintain accurate records within the agency's scheduling and documentation software.

Client Care Coordination

Conduct in-home client assessments and assist with new client admissions.
Introduce caregivers to new clients and ensure a smooth transition into care.
Perform routine client satisfaction and supervisory visits.
Address client concerns and coordinate service adjustments as needed.

Recruitment & Onboarding

Assist with recruiting, interviewing, hiring, and onboarding caregivers.
Coordinate orientation and ensure all required employment documentation is completed.
Monitor employee performance and assist with ongoing coaching and support to ensure that clients are receiving the highest level of support services and continuity of care.

Compliance & Quality Assurance

Maintain employee and client files to ensure compliance with state regulations and agency policies.
Track caregiver credentials, certifications, driver's licenses, and other required documentation.
Assist with audits and quality assurance initiatives.

On-Call Responsibilities

Participate in a rotating weekend on-call schedule.
Respond promptly to urgent caregiver call-offs, client concerns, and scheduling emergencies.
Fill-in for unstaffed client shifts if there is a change or cancellation in a client or caregiver schedule.

Required Qualifications

Active Texas Certified Nursing Assistant (CNA) certification (Required)
Minimum 5 years of home care experience (Required)
Reliable transportation, as well as the ability and willingness to travel locally for client visits (Required)
Previous experience in home care scheduling, staffing, or healthcare operations preferred.
Strong organizational and multitasking skills.
Excellent verbal and written communication skills.
Ability to remain calm and make sound decisions under pressure.
Proficient in Microsoft Office (Word, Excel, Outlook).
Experience using home care scheduling or CRM software preferred (CareSmartz360 experience is a plus).

Required Hands-On Caregiving Experience

Candidates must have a minimum 5 years of caregiver experience and be comfortable performing or demonstrating:

Personal care and bathing assistance
Incontinence care
Safe transfers and gait belt assistance
Hoyer lift operation
Dementia care
Meal preparation and light housekeeping
Mobility assistance
Hospice care
Other essential non-medical home care tasks

Preferred Qualifications

Familiarity with private pay home care operations, including client intake, service coordination, scheduling, and family communication.
Knowledge of long-term care insurance (LTCI) processes, including benefit verification, claims coordination, and communicating with families regarding coverage requirements.
Experience with CareSmartz360 or other home care scheduling software.
Knowledge of Texas home care regulations and compliance requirements.
Bilingual in English and Spanish preferred.
