Full Job Description

Are you a compassionate and skilled social services professional who thrives on helping vulnerable adults maintain their independence, dignity, and quality of life? Do you enjoy building meaningful relationships, solving complex challenges, and collaborating to create life-changing outcomes?

We are seeking a dedicated Care Manager to join our team. In this rewarding role, you will work directly with older adults and individual with disabilities transition out of hospitals and care facilities so they can live independently. Through comprehensive assessments, care planning, advocacy, and ongoing support, you will play a critical role in helping clients remain safely and successfully in their homes and communities.

What You'll Do

Conduct comprehensive assessments and ongoing reassessments of clients' medical, psychosocial, mental health, environmental, and support needs.
Develop individualized, client-centered care plans and coordinate services to meet identified needs.
Conduct home visits, hospital visits, skilled nursing facility visits, and accompany clients to appointments when clinically appropriate.
Connect clients with community resources, benefits, and entitlement programs, including Medicare, Medi-Cal, and other supportive services.
Provide crisis intervention, advocacy, problem-solving, and therapeutic support.
Monitor service delivery and ensure clients receive timely, high-quality care.
Maintain regular contact with clients and support networks to evaluate progress and adjust care plans as needed.
Collaborate with physicians, behavioral health providers, community agencies, caregivers, and interdisciplinary team members.
Document case management activities accurately and timely to ensure continuity of care.
Participate in quality improvement initiatives, team meetings, training, and professional development opportunities.
Continuously assess clients' ability to remain safely in the community and coordinate higher levels of care when necessary.

Education

One of the following qualifications is required:

Master's degree in Social Work (MSW), Public Health (MPH), Marriage and Family Therapy (MFT), Psychology, Counseling, Gerontology, or a related field

Bachelor's degree in Social Work or a related field plus a minimum of two (2) years of relevant social work or case management experience, including demonstrated ability to independently conduct biopsychosocial assessments, identify care needs, develop interventions, and manage complex cases.

Experience & Skills

Minimum one year of experience working with older adults and/or adults with disabilities
Strong understanding of medical, behavioral health, and psychosocial challenges affecting functionally impaired adults
Experience supporting individuals with mental health diagnoses and/or substance use disorders highly preferred
Knowledge of community-based healthcare systems and case management practices
Excellent interpersonal, communication, and relationship-building skills
Strong critical thinking, problem-solving, and organizational abilities
Ability to manage multiple priorities in a fast-paced environment
Proficiency with electronic documentation systems and Microsoft Office applications
