Full Job Description

Qualifications: Must be a graduate of an accredited school of nursing. The registered nurse must possess a current Missouri nursing license which is in good standing. The nurse must exhibit the necessary knowledge, skills, and experience.

Working Conditions: The employee will have periods of walking, standing, and sedentary work. Must be able to drive own personal car with automobile insurance. The employee will be subjected to and go out in all weather conditions. Must be exposed to all patient elements and home environments, including pets. The employee will be subject to stress, frustration, and fatigue.

Brief Summary: Provide professional nursing care for an assigned patient according to established standards and practices. The position requires a nurse who is well trained in all aspects of patient care, has the ability to make sound professional judgments, good clinical assessments, and provide therapeutic treatment of patients. Responsibilities include direct patient care, teaching patients and families, and supervision of other nursing personnel.

Duties and Responsibilities:

Make the initial evaluation visit.
Establish and initiate patient clinical record, an individualized plan of care with interventions and patient-centered goals.
Establish medication sheet, reconciliation of medications, and keep medications up to date, including prescription, over-the-counter, oxygen, and supplements.
Keep medication list in chart and home up to date.
Regularly evaluate the patient’s needs.
Complete clinical and progress notes.
Maintain current documentation in patient clinical records, including but not limited to: 485 plan of treatment, interim physician orders, case conference notes, communication notes, calls to physicians, OASIS, discharge and transfer summaries, aide care plans, and medication records.
Document all care given, teaching completed, coordination of care instituted, and calls to physician for changes in patient condition.
Regularly evaluate the patient’s needs and progress toward goals.
Develop the aide care plan, keeping it up to date with any changes, communicate with the aide on care plan and changes, and supervise the aide every 14 days or less.
Confer with LPN as needed to communicate goals and interventions, and supervise LPN.
Accept, record, and transcribe verbal and phone orders from physicians.
Attend required meetings, in-services, and educational programs.
Keep up to date on ever-changing regulations.
Maintain professional growth and development through seminars, workshops, and professional affiliations.
Comply with established nursing standards. Implement and maintain established department and hospital policies, procedures, objectives, quality assurance, safety, environmental, and infection control protocols. Interpret these to subordinate personnel, patients, and families as appropriate.
Follow established routines and utilize supplies and equipment in a cost-effective manner.
Charge for supplies used or left in the patient’s home at each visit.
Schedule patients in a cost-effective manner to decrease mileage.
Maintain courteous communication and project a positive and helpful attitude toward patients, caregivers, visitors, medical staff, the public, and fellow hospital employees.
Report any unusual symptoms, complaints, or problems to the administrator of home health.
Perform other duties as directed or required.

Duties

Conduct comprehensive patient assessments including vital signs, physical examinations, and detailed health histories to determine individual care needs.
Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols.
Provide specialized treatments such as wound suturing, ostomy care, catheterization, and management of complex medical devices like ventilators and dialysis equipment.
Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques to enhance recovery and quality of life.
Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems like Epic or Cerner to document care plans, progress notes, and compliance with HIPAA regulations.
Coordinate discharge planning and case management for post-acute care settings including hospice & palliative medicine, memory care, or long-term care facilities.
Triage patient concerns over the phone or in person to determine urgency and appropriate level of care; respond effectively in emergency situations such as acute pain episodes or trauma cases.

Skills

Proven experience with home health nursing practices across diverse settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics.
Strong knowledge of medical terminology, physiology, ICD coding (ICD-10/ICD-9), CPT coding, and diagnostic evaluation procedures.
Skilled in performing invasive procedures including IV insertion, phlebotomy, laboratory specimen collection/processing, and sterile dressing changes.
Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation and compliance with NCQA standards.
Expertise in managing complex cases involving infectious disease care, acute pain management, behavioral health issues, geriatrics including dementia & Alzheimer’s care.
Ability to work independently while maintaining excellent communication with healthcare teams; experience triaging urgent situations such as trauma medicine or ICU-level emergencies.
Knowledge of Workers' Compensation law related to occupational health cases; experience working with people with disabilities or developmental disabilities preferred.
