Job Description: Professional Social Worker/Case Mangement/Behavioral Health

Provide social services and assistance to improve the social and psychological functioning of adults, children, and their families. Social Workers and Case Managers in this field assess their clients’ needs and aid in improving their situation. This often includes coordinating available services to assist a child or family.

Duties:

· Uses appropriate tools to make assessments and evaluations; prepares summaries and makes recommendations regarding program services; communicates information to the treatment team; maintains knowledge of updated program changes; maintains knowledge of ASAM and DSM-5 patient placement criteria for appropriate placement and continuing care.

· Screening – Conduct interviews and assessments for outpatient treatment applicants.

· Intake – Conduct an initial assessment and complete all required documentation upon admission.

· Orientation – Describe the service program and treatment to new clients.

· Assessment – Gather data from the client and other sources, analyze and interpret it to identify the client's strengths, weaknesses, and needs.

· Treatment Planning – Collaborate with the client to develop treatment plans, identifying and prioritizing problems, goals, and treatment methods.

· Counseling – Provide individual, family, and group counseling.

. Education – Provide clients and their families with information about substance abuse, co-occurring disorders, treatment, recovery, and resources.

· Case Management – Coordinate with clients and resources to focus on identified needs and goals.

· Experience or familiarity with the IM+CANTS Strengths and Needs Assessment tool.

· Referral – Identify client needs that can’t be met at K.A.M Alliance and assist the client to use community resources and support systems.

· Consultation – Relate with professionals on client treatment and services to assure comprehensive care. Prepare for and participate in individual and group supervision, case review, and clinical staff meetings

· Record Keeping – Maintain accurate and up-to-date counseling records.

· Develop treatment plans with clients, encouraging them to create and pursue goals for improving their lives.

· Provides services to assist family members who suffer from addiction or other mental health issues.

· Identifies potential conflicts, implements intervention techniques to prevent or intervene in potential crises, and assists families that have serious domestic conflicts.

· Develops and maintains appropriate professional boundaries between self, staff, members, and collaterals and identifies issues related to ethics and boundaries, as identified in the Code of Ethics policy. Adheres to confidentiality requirements.

· Maintains appropriate credentials and pursues internal and/or external training and development opportunities.

· Performs related duties as assigned and attends mandatory in-services.

· May be required to travel to schools, hospitals, or other treatment care facilities.

Skills/Qualifications:

· Multi-tasking, Flexibility, Telephone Skills, Customer Service, Time Management, Organization, Attention to Detail, and Scheduling

· Comfortable and proficient with using a computer, electronic medical record (EMR), or Telehealth Program (ZOOM, telephonic, and video programs), its software, and all other office equipment.

· Professionalism, Quality Focus. Ability to work remotely (tele-health video)

· Must be proficient in Word, Excel, PowerPoint, and Publisher computer programs.

· Must possess excellent communication, organizational, and interpersonal skills

Required Education:

· High school diploma or GED and 12 credit hours of college education in substance abuse counseling and/or other human services area.

· Master’s or higher degree required. Higher education is given preference

Required certification or license:

· Licenses/Certificates (MSW, LSW, LCSW, LCPC, QMHP, MHP, Certified Counselor or above).

· CPR and First Aid Certification, preferred.

Minimum experience required:

Two years of experience in a substance abuse treatment, correctional, or other outpatient social service/mental health setting. The two-year requirement can include time in an approved supervised practicum or internship program.
