Full Job Description

Community Behavioral Health Center (CBHC) has served Northeast Ohio for more than 30 years, and we're growing. We're hiring compassionate, motivated Arabic-speaking Community Mental Health Case Managers (QMHS) to join our multidisciplinary team and help adults with serious mental illness live more independent, stable lives.

What You'll Do

Conduct comprehensive assessments of each client's strengths, needs, preferences, and goals
Build strong therapeutic relationships that support recovery and independence
Coordinate medical, behavioral health, housing, vocational, financial, and community services
Help clients navigate healthcare systems and connect with community resources
Advocate for clients to ensure timely access to services and supports
Collaborate with the treatment team to coordinate and monitor client care
Communicate with family members, healthcare providers, and community agencies as needed
Educate clients about available benefits and community resources
Maintain accurate, timely clinical documentation and billing records
Protect client confidentiality and comply with HIPAA, Medicaid, and agency policies

What We're Looking For

Required:

Bachelor's degree in Social Work, Psychology, Human Services, Sociology, or a related field
Fluency in Arabic and English (spoken and written)
Valid Ohio driver's license and reliable personal transportation
Ability to travel throughout the community to meet with clients
Strong organizational, communication, and documentation skills
Ability to work independently while collaborating with a multidisciplinary treatment team

Preferred:

Experience working with individuals living with serious mental illness
Knowledge of Medicaid-funded behavioral health services
Familiarity with community resources and public benefits, including SSI and SSDI
Previous case management or community behavioral health experience
