Full Job Description

Family Case Management positions available in two Gladden programs.

Family to Family Program: the program serves families referred by FCCS for additional case management services and linkage to resources. In addition to individual case management, the team plans social and educational events for both the youth and families in the program. Families are allowed to remain in the program for up to a year. Traditional files and case notes are required. Attention to detail in file documentation for monthly billing. This position also requires driving the agency vans to events and activities. The Family to Family team consists of 4 case managers.

Homelessness Prevention Program: the program serves families referred from the Homeless Hotline who have lost their housing and are temporarily staying with friends or family members. They are linked to a case manager in the HP program who will provide needed resources such as employment resources. Upon enrollment families understand that is their responsibility to find their housing. The case managers provide housing resources and education. Once a family is approved for housing, the HP case manager can apply for first month's rent and deposit through the DCA process which is reviewed and approved by The Community Shelter Board. Families are in the program for an average of four months. Once they are housed and linked to additional resources such as utility, school transportation,and furniture, they are closed out of the program. The HP team consists of 8 case managers.

Full-time positions: $47,000

Both positions have a strong team approach. Caseloads run 20-25. BSW or related degree required, LSW preferred. Extensive knowledge of community resources. Current driver’s license, insurance, and the ability to travel as necessary and transport clients. Background check and drug test.

Annual benefits include 11 paid holidays, 3 personal days, 12 sick days,and 20 vacation days.
