Full Job Description

POSITION SUMMARY

The Quality Coordinator will work under the supervision of the Quality Improvement Manager to ensure Polara Health provides high quality integrated care services in a professional manner which aligns with all federal, state and local regulatory guidelines. The Quality Coordinator plays a role in meeting annual quality improvement plan goals. The Quality Coordinator will act as a source of validity, legality and goal achievement with direct care staff as the Complaint Liaison. The Quality Coordinator is responsible for assisting with the review and submission of all Incident Reports to external parties as required and is the liaison for the grantors and aid to Quality and Risk regarding all Quality-of-Care concerns.

ESSENTIAL RESPONSIBILITIES

Displays adherence to Polara Health's mission and vision as well as departmental goals for continuous quality improvement.

Supports all organizational, state, and federal policies and regulatory guidelines.

Collaborate with internal and external partner supervisors and staff for improved communication and efficiency in workflow.

Completes reports regarding complaints and grievances for data analysis and improvement planning.

Performs quality assurance functions to accomplish organizational-wide coordination, monitoring, and reporting of quality-assurance studies according to the QA plan.

Informs director of any new and/or revised QA regulations.

Assists departments with the coordination of audit information and recommends appropriate data-gathering mechanisms, procedures, etc.

Creates a satisfactory working environment with other departments performing quality-assurance studies, and assists with the accumulation, display, routing, and dissemination of information to appropriate committees, physicians, and departments.

Assists the Director and manager with revisions to the QA plan for review.

Assists with developing and maintaining an unusual occurrence procedure.

Assists with the monitoring of unusual occurrences; prepares action-taken reports for the committee for the Director of Quality and Risk.

Maintains confidentiality and excellent communication skills with partner agencies, clients and staff to provide conflict resolution and attain excellent customer service.

Investigates and documents resolution with respect to the rights and wishes of clients, patients, visitors and Polara Health employees.

Maintains all reporting responsibilities regarding Critical Incident Reports.

Collaborates with staff and participates in all assigned committee meetings to aid in process improvement and brainstorming.

Performs other duties as assigned.

QUALIFICATIONS

21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member’s scope of practice; or minimum 18 years old for select administrative positions

Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card

Must possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization

Minimum of a High School Diploma or GED required. Associate’s degree preferred, or 2 years of professional experience. (Preferred LVN or RN license in good standing)

If licensed, maintains professional requirements and is willing to provide/train for Supervision as needed.

Proficient with data management, analysis and utilization of MS Office Excel.

Excellent analytical and problem-solving skills.

Excellent communication skills, both verbal and written.

Strong people skills to engage staff as department liaison.

Ability to navigate electronic medical records and other databases.

Self-starter who is motivated, energetic, and enthusiastic.

Ability to handle frequent interruptions.

Ability to maintain close attention to detail and accuracy.
