Stanley Healthcare Center is seeking an Social Service/Activity Director.

Clinical/Administrative:

Ensures the residents psychosocial and concrete needs are identified and met in accordance with Federal, State and Company requirements.
Orients residents to the center.
Discharge planning.
Implements and updates Resident Care Plan and Social History.
Maintains records of outside referral.
Assists in coordinating residents financial affairs.
Communicates needs and Care Plans to resident, family, responsible party and appropriate staff.
Participates in meetings, in-services, and training as required.
Meets with consultants as required.
Organizes and facilitates Family Group.
Organizes and conducts New Admissions Group.
Plans and Implements activities for all residents in accordance with Federal, State and Corporate requirements and Policies.
Publishes a monthly Newsletter according to policy and procedure.
Documents in writing residents progress and/or response to activities in Patient Health Record as required.
Maintains written records of residents attendance at activities.
Decorates Center to reflect current season or holiday.
Maintains inventory of departmental supplies and equipment.
Documents in writing resident progress and or in person with community agencies, organizations, and or churches.
Participates in promotional and marketing plans.

Consumer Service:

Presents professional image to consumers through dress, behavior, and speech.
Adheres to Company standards for resolving consumer concerns.
Ensures that all consumer and patient rights are protected.

Supervision:

Meets department work goals through proper assignment of staff.
Monitors staff performace through coaching, praising, and recognizing effective performance or takes progressive diciplinary action after coaching as needed.
Evaluates quality and quantity of department services accomplished by staff.

Qualifications:

Written and verbal English communication skills required for business necessity.
knowledge of social resources in the community
Good interpersonal skills necessary.
Good administrative and organizational skills necessary.
Experience in a long-term care setting or related healthcare field required.
High School Diploma or Equivalent.
Degree in Psycho/Social Health preferred.

Pay: $22.00 - $27.00 per hour
