Case Manager Residential Treatment

Position Title: Case Manager Department: Clinical Reports To: Director of Case Management and Clinical Director

Overview: Monarch Mental Health is committed to providing comprehensive and client-centered care for individuals transitioning from various settings, including state hospitals and other institutions, to community living. Monarch Mental Healthoffers a supportive environment where clients can reintegrate into society with the necessary therapeutic support. The Case Manager plays a pivotal role in ensuring clients receive coordinated and effective services tailored to their unique needs and goals.Key Responsibilities:

Client Assessment and Planning:

Conduct comprehensive assessments of clients' needs, strengths, and goals using evidence-based tools and techniques.
Develop individualized service plans in collaboration with clients, families, and the clinical team, ensuring goals are specific, measurable, achievable, relevant, and time-bound (SMART).
Regularly review and update service plans to reflect clients' progress, changing needs, and new goals.

Coordination of Services:

Coordinate and facilitate access to necessary services, including medical, psychological, social, educational, and vocational resources.
Serve as a liaison between clients, their families, and service providers to ensure continuity and integration of care.
Advocate for clients’ needs and rights within the community and service systems, ensuring they receive equitable and appropriate services.

Monitoring and Evaluation:

Monitor clients' progress and engagement with services through regular follow-ups, home visits, and assessments.
Document clients' progress, case notes, and any changes in status or needs accurately and promptly in the case management system.
Evaluate the effectiveness of service plans and interventions, making adjustments as needed to optimize outcomes.

Support and Crisis Management:

Provide supportive counseling and guidance to clients, helping them navigate challenges, build coping skills, and enhance resilience.
Respond to crises and emergencies, providing appropriate interventions, support, and follow-up care.
Develop and implement crisis management plans in collaboration with the clinical team, ensuring staff are trained in crisis intervention protocols.

Collaboration and Communication:

Work collaboratively with the clinical team, providing input and feedback on client progress, needs, and barriers to care.
Communicate effectively with clients, families, and other stakeholders, fostering a supportive and therapeutic environment.
Build positive and supportive relationships with clients, promoting trust, engagement, and adherence to service plans.

Resource Development and Utilization:

Identify and develop relationships with community resources, service providers, and support networks.
Maintain an updated directory of resources and services available to clients, ensuring comprehensive and timely access to support.
Assist clients in applying for and accessing community resources, benefits, and entitlements, such as housing assistance, Medicaid, and food stamps.

Compliance and Documentation:

Ensure compliance with all relevant regulatory standards, accreditation requirements, and organizational policies.
Maintain accurate and timely documentation of all case management activities, client interactions, and service outcomes.
Participate in regular staff meetings, training sessions, and professional development opportunities to enhance skills and knowledge.

Qualifications:

Education and Certification:

Bachelor’s degree in Social Work, Psychology, Counseling, or a related field required; Master’s degree preferred.
Certification as a Case Manager (CCM or equivalent) preferred.
Basic Life Support preferred

Experience:

Minimum of 2 years of experience in case management or a related role, preferably in a mental health or residential setting.
Experience with crisis intervention and de-escalation techniques.

Skills:

Strong interpersonal, communication, and advocacy skills.
Ability to work effectively in a multidisciplinary team and independently.
Excellent organizational, time-management, and problem-solving abilities.
Proficiency in using case management software and other relevant technology.
