POSITION SUMMARY
The Lead Case Manager will monitor and provide assistance to Case Managers and upper management under a combination of administrative/office duties and field work related to our housing programs. This position is also responsible for case management services for those with a dual diagnosis and experiencing homelessness, and supporting the overall program with the Program Director of the department. This position will be responsible for approximately 15 clients/individuals or a head of household with a family, and some administrative and program coordination activities. The position will be supervised by the Director of Supportive Housing.
DUTIES AND RESPONSIBILITIES
PROGRAM COORDINATION
- Support the data tracking of the Client Master List, openings in each program and represent HPP within the PPCOC Coordinated Entry system.
- Provide quality assurance on client files and data tracking in HMIS; review intakes and applications before submission to the Director of Supportive Housing for approval.
- Supervise an intern or volunteer assigned to the program and provide direction and training of Policies and Procedures.
- Provide mentoring and guidance to the housing Case Management team, support the Director of Supportive Housing when out of the office. Areas of support may include but not limited to, preparing rental check request(s), handling client requests and other general team responsibilities.
- Coordinate monthly client luncheons, welcoming meetings, and monthly newsletter for clients.
- Assist supervisor with training new and incumbent staff, evaluate training needs, implement policies and procedures, and provide feedback to the supervisor.
HOUSING
- Meet with prospective residents to assess and/or determine eligibility; perform intake and screening services, conduct review/utilize results from VISPDAT to (HUD tool) to aid in pre-selection process.
- Work with the housing team and street outreach program to locate the identified person, work on intake and application process.
- Guide applicants in apartment/housing application process; obtain necessary documentation and referrals for and/or with assistance of application.
- Coordinate communication regarding housing needs between client and Property Manager.
- Provide assistance in procuring furniture, home furnishings, etc. when needed and to assist with moving into a home.
- Conduct annual recertifications and attend annual inspections and lease signings.
CASE MANAGEMENT
- Develop individualized service plans for clients and assess monthly client engagement and wellness and make appropriate services available as needed.
- Conduct home visits with each client at a minimum of once a month; maintain weekly contact by phone; work with each to set goals for healthy individual growth with an emphasis on maintaining recovery; monitor progress in attaining goals; monitor adherence to all Homeward Pikes Peak – Housing First policies and procedures.
- Provide transportation for clients to appointments when necessary and/or connect clients to community transportation resources.
- Coordinate with other community service agencies to access service for clients as needed.
- Maintain accurate records, which include client charts as well as internal and external reports; maintain confidentiality and privacy per standards with records.
- Contribute to monthly outcome reports.
- Enter data into HMIS and other outcome tracking tools and client files & adhere to data quality goal (90%).
- Participate in the following as needed: Coordinated Entry activities; CHAP; RMHS VFAH (working with veterans).
- Perform other duties as assigned.
HOMEWARD PIKES PEAK TEAM RESPONSIBILITIES
- Attend and participate in staff meetings and monthly client luncheons.
- Attend / participate / interact with donors, attend community events, or fundraising as requested.
- Participate in weekly supervision.
- Integrate with the overall organization and collaborate with the community.
- Produce documentation timely as requested for audits and audit resolutions.

KNOWLEDGE, SKILLS, ABILITIES:

- Proficiency in general computer skills and Microsoft Office applications including Word, Excel, and Outlook; word processing, spreadsheets, CRM databases, and other case management platforms.
- Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
- Demonstrated knowledge and training in generalized health care standards, substance abuse recovery process and treatment, mental health care, and trauma informed care skills.
- Knowledge of basic care coordination, medical/hospital and/or community resources.
- Ability to work comfortably among clients with or without substance abuse disorder, who are referred from city and county parole/probation departments, department of social services, and other relevant agencies.
- Ability to act effectively as a liaison with other agencies that support HPP clients.
- Ability to use critical thinking to prioritize, make analytical decisions, and problem solve.
- An understanding of issue related to homelessness, mental illness and substance abuse.
- Detail oriented and organized, and ability to maintain confidentiality.
- Ability to maintain professional boundaries with clients.
- Self-starter and ability to work independently with minimal supervision.
- Ability to occasionally lift or move up to 25 lbs.
MINIMUM QUALIFICATIONS:
- Bachelor’s level degree in Social Services required.
- 3 – 5 years of experience in the human service field providing direct client supportive services.
- First Aid / CPR certified, or willing to obtain within 3 months of hire.
- Experience working directly with people from diverse socioeconomic and ethnic backgrounds, ability to communicate effectively, crisis intervention, and harm reduction.
- Understanding of regulations and standards regarding privacy and confidentiality in the provision of counseling services.
- Demonstrated experience related to the essential duties, knowledge, skills, and abilities of a Housing First Case Manager preferred.
- Must be proficient in record keeping, tracking, maintaining, and reporting statistics according to housing program guidelines and requirements.
- Valid Colorado driver’s license, clean driving record, and must carry personal vehicle insurance.
PREFERRED QUALIFICATIONS:
- Three (3) years of supervisory or management experience preferred.
- Lived experience is preferred, with an understanding of challenges related to homelessness, mental illness, and substance abuse.
- CAS (Colorado Addiction Counselor) or CAT (Certified Addictions Tech) certification is preferred.
- Knowledge of Mental Health Diagnosis and addictions counselling, experience or certifications (such as LAC) preferred.
- Experience working for a non-profit organization.
- Spanish speaker preferred but not required.
PHYSICAL DEMANDS
While performing the duties of this job, the employee is frequently required to sit at a desk or conference table; and talk or hear when interacting with various individuals and groups. The employee is occasionally required to stand while conversing with various individuals; and walk throughout the office, rental property, park and to/from vehicles and retail locations. The employee occasionally is required to use hands to finger, handle, or feel to operate the computer or telephone; may be required to stoop, kneel, or crouch to communicate with residents and to place items in or get items from low drawers or shelves. The employee must frequently lift and/or move up to 25 pounds, which is generally office supplies such as reams of paper, files, and forms, and may include small furniture items occasionally.

While performing the duties of this job, the employee may be exposed to airborne viruses, bacteria, or other bodily pathogens carried by residents or present in clients’ homes. The employee is occasionally exposed to outside weather conditions and fumes or airborne particles when driving from one HPP location to another or participating in an outdoor resident social activity. The noise level in the work environment can range from quiet while in a private office to loud while driving, attending a large residential social event, or visiting resident common areas where there are televisions and equipment operating, phones ringing, light to heavy traffic, and people talking.
COMPENSATION AND BENEFITS
The pay range for this position is $55,000 – $60,000 annually; this is a full-time, exempt position.
