MISSION

NeighborWorks Housing Solutions is a regional nonprofit organization dedicated to affordable housing, homelessness prevention, and community development. We believe everyone deserves a safe, stable place to call home and strive to empower families and individuals through housing and supportive services.

SUMMARY

NeighborWorks Housing Solutions (NHS) is seeking a compassionate and motivated Housing Stabilization Case Manager to support families who have recently transitioned from shelter into permanent housing. The Case Manager provides strengths-based, trauma-informed stabilization services for up to 36 months, helping clients sustain housing, increase income, and access essential supports. This position is based within a supportive housing program that serves families from diverse backgrounds and require additional guidance and resources to achieve long-term stability. This role reports to the Stabilization Manager and works closely with a collaborative team of HomeBASE Case Managers and service providers.

PROGRAM OVERVIEW
The program provides intensive housing and recovery support services, helping families build stability and independence.

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Provide client-centered, trauma-informed case management services aligned with NeighborWorks Housing Solutions' mission and values.

· Uphold agency standards of professionalism, ethics, boundaries, and confidentiality.

· Ensure adherence to program policies and procedures and integrate Housing First principles into daily practice.

· Assist clients with housing search and stabilization, including communicating with landlords, viewing units, and securing leases.

· Support clients with the logistics of moving into housing, including accessing furniture, household items, and food.

· Actively coordinate care with recovery programs, medical providers, housing authorities, and community-based organizations.

· Facilitate client access to resources such as employment services, education/training programs, childcare, and public benefits.

· Promote housing retention by supporting clients with tenancy education, utility management, timely rent payments, and prevention.

· Conduct regular home visits and community-based meetings with clients to monitor progress and provide ongoing support.

· Develop individualized service plans with clients, setting measurable and time-bound goals for self-sufficiency.

Accompany clients to appointments and hearings, and provide advocacy as needed.

· Maintain clear and timely documentation, including assessments, case notes, and funder-required reports.

· Participate actively in staff meetings, supervision, and professional development trainings.

QUALIFICATIONS AND CERTIFICATIONS

· Bachelor’s degree in social work, psychology, human services, or related field preferred. Relevant experience will be considered in lieu of a degree.

· 2+ years of experience in housing stabilization, case management, or related work.

· Knowledge of supportive housing models, including Housing First and Rapid Re-Housing, preferred.

· Experience working with individuals in recovery and/or survivors of domestic violence is strongly preferred.

· Strong interpersonal and communication skills; ability to build rapport and trust with diverse populations.

· Valid driver’s license and access to reliable transportation required.

· Bilingual in English and Spanish and/or Haitian Creole a plus.

PHYSICAL DEMANDS/WORKING CONDITIONS

This position involves field-based inspection work and requires the ability to work independently. The role includes frequent travel to inspection sites and the use of remote systems for reporting and communication. Successful candidates must be self-motivated, well-organized, and comfortable managing tasks both in the field and in an office environment. This requires the ability to drive regularly to various field locations. Inspectors must be able to walk, stand, bend, climb stairs, and navigate uneven terrain as part of property or site inspections. Occasional lifting of materials and equipment up to 25 pounds may be necessary.
