Job Description

Performs moderately complex (journey-level) to advanced (senior-level) case management work with a primary focus of acting as a liaison between TJJD and local juvenile probation departments providing post-commitment services to youth pending admission to a TJJD operated facility and helping ensure probation departments have the information and resources needed to provide appropriate services. Following commitment of the youth by a juvenile court, TJJD facilitates the intake, monitoring and transition of youth pending admission into one of the TJJD operated facilities which requires collaboration between TJJD and local juvenile probation department to facilitate the exchange of information about the youth¿s case and scheduling of an intake date. Youth remaining in pre-adjudication facilities, pending admission to TJJD, may need services provided by juvenile probation departments, in collaboration with TJJD, to begin rehabilitation and may include psychological and criminogenic risk and needs assessments, intake screenings, medical services, mental health crisis intervention, pre-treatment behavior monitoring, education services, and general rehabilitative concepts foundational to treatment completion in a TJJD operated facility.

Work involves reviewing case files; participating in the intake and orientation of youth; collaborating in the development of individual case plans (including goal development) and reviewing monthly progress reports with facility staff, youth, and their family members or guardians; evaluating outcomes and providing technical assistance, guidance, and training as needed; coordinating medical treatment, and providing needed referrals; verifying services provided for grant disbursements; and coordinating intake, admission and transition planning with youth, their family members or guardians, facility staff, community providers, and parole officers to ensure a continuity of service delivery throughout the youth¿s intake and admission to TJJD. Works with service providers to understand and deliver the tenants of The Texas Model.

May be required to work in excess of 40 hours per workweek, different shift patterns, and be on-call. Regular travel may be required based on location of program assignment.

Career Path Promotion Opportunity: The TJJD Admission Case Management Specialist job series has been established within the TJJD Case Manager career path. An employee is hired at a level corresponding with the career path education and experience requirements and may promote up to Case Manager IV without competing for a vacant position after the employee meets: (1) the education and experience requirements for the next higher level; and (2) the established performance criteria per agency policy (PRS.15.07).

Pre-Service Training Academy Requirement: Newly hired case managers or rehired case managers who separated more than 18 months will be required to attend the pre-service training academy. Case managers who have worked for TJJD and return to employment within 18 months of their separation date are not required to attend the pre-service academy and will be placed in full-time status on their date of employment.

Levels of Work Performed by Career Path Level:

Case Manager II: Performs moderately complex (journey-level) work under general supervision with moderate latitude for the use of initiative and independent judgment.

Case Manager III: Performs highly complex (senior-level) work under limited supervision with considerable latitude for the use of initiative and independent judgment.

Case Manager IV: Performs advanced and/or supervisory (senior-level) work under minimal supervision with extensive latitude for the use of initiative and independent judgment.

Essential Job Duties

Reviews the youth¿s master file and ensures all orientation and assessment documentation is complete; requests missing information; and shares appropriate information discovered during master file reviews with facility staff; collaborates in the development of rehabilitation goals for case planning; ensures completion of appropriate intake assessments and screenings; and facilitates continuity of care during transition of youth into TJJD operated facilities.

Conducts ongoing monitoring of services and the youth¿s behavior; verifies the provision of services by the local juvenile probation department, youth attendance and engagement in provided services; coordinates required documentation for youth admission and movement into a TJJD operated facility; facilitate quality collaboration; verifies accuracy of grant disbursement reports; assists with preparing materials in response to requests for grant program information; address needs as they emerge; and ensures continuity of services as youth transition into a TJJD operated facility.

Serves as liaison between TJJD and the pre-adjudication facility, youth family members and advocates, community and professional groups, and the general public; consults with departmental management and facility staff to resolve problems, identify training needs, and discuss program effectiveness; recommends improvements to processes and assists with developing action plans to improve processes; and provides guidance to facility staff in the development and integration of new TJJD methods and procedures.

Communicates with youth, family members or guardians, and facility staff at least monthly, or as needed, to ensure proper understanding and effective implementation of rehabilitative goals, including the development of goals to address identified risk and needs and teaching basic rehabilitative concepts; actively participates in and prepares for such meetings; provides guidance and direction as needed; and ensures access to appropriate resources.

Coordinates healthcare activities to facilitate youth access to medical and psychiatric services and to help ensure the medical and medication needs of youth are met while assigned to the residential facility and the continuity of applicable medical care upon transition.

Coordinates providing information to TJJD intake and facility treatment staff regarding the youth¿s rehabilitation needs, behavior and progress during the youth¿s pre-admission, and needs for continuity of treatment and care.

Coordinates transportation, as needed, for youth to transition into a TJJD operated facility.

Collaborates with departmental staff in meeting established objectives and goals; shares information; identifies concerns and trends; and recommends improvements to resolve problems or enhance operations; participates in staff development, including the TJJD approved treatment program, Case Management Standards (CMS) and Correctional Care System training.

Assists the primary case manager with planning for intake and transition into a TJJD operated facility; creates transition plans and updates to facilitate continuity of treatment and care; identifies appropriate services relating to transition; coordinates and submits needed referrals; and works in collaboration with the transition staff to ensure successful intake and admission to TJJD.

Performs a variety of related duties not listed, to be determined and assigned as needed.

Performs all duties in compliance with agency safety policies and procedures:

Reports safety hazards and corrects hazards when possible.
Completes required documentation in the event of an accident/injury within requested timeframes.

Attends work on a regular and predictable schedule, and on an as-needed basis, in accordance with agency attendance and leave policies.

Actively embody and support the Texas Model by demonstrating its core values in daily tasks and interactions with others.

Minimum Qualifications

Requirements for Case Manager II ¿ Pay Level One (1):

Unrelated Bachelor¿s degree. No experience required.

High School Diploma or equivalent, and

Fifteen (15) semester hours of related course work, and

Two (2) years full-time wage-earning experience consisting of any combination of the following: case management work or primary service responsibilities in a residential setting for youth including assessments, development and implementation of individual case plans, development/coordination of services, and placement recommendations and referrals of youth; social work; or Texas Juvenile Justice Department (TJJD) / Texas Youth Commission (TYC) Juvenile Correctional Officer (JCO) / Youth Development Coach (Coach) experience.

High School Diploma or equivalent, and

Less than fifteen (15) semester hours of related course work, and

Five (5) years full-time wage-earning experience consisting of any combination of the following: case management work or primary service responsibilities in a residential setting for youth including assessments, development and implementation of individual case plans, development/coordination of services, and placement recommendations and referrals of youth; social work; or Texas Juvenile Justice Department (TJJD) / Texas Youth Commission (TYC) Juvenile Correctional Officer (JCO) / Youth Development Coach (Coach) experience.

Requirements for Case Manager III ¿ Pay Level Two (2):

Bachelor¿s degree with major in Clinical Psychology, Psychology, Sociology, Social Work, Family Relations, Guidance and Counseling, Rehabilitation, Criminal Justice-Corrections (not Law Enforcement or Police Sciences), Education (or education/teacher certification), or another related social sciences field. No experience required.

Requirements ¿ All Levels:

Must have a valid driver license.
Must be at least 21 years of age.

Note: Any degree, diploma, high school equivalent, or course hours required to meet minimum qualifications must be obtained from an accredited educational institution.

Requirements for continued employment:

Maintain a valid driver license.
Failure to maintain required licensure/certification will result in termination of employment.
Obtain and maintain certification in TJJD¿s Use of Force techniques, which requires physical restraint of juveniles.
Obtain and maintain certification in First Aid and CPR.
Failure to maintain required certification will result in termination of employment.

Convicted or received deferred adjudication for domestic violence involving a family member (including non-spouse partners).
Convicted or received deferred adjudication for official oppression.

Outstanding warrant.
