What You'll Do

As Clinical Manager, you'll provide leadership, mentorship, and clinical oversight while supporting a talented interdisciplinary team committed to delivering exceptional patient care.

Your responsibilities will include:

Lead, mentor, and inspire nurses, therapists, social workers, and clinical staff
Foster a culture of teamwork, accountability, and clinical excellence
Ensure compliance with Medicare Conditions of Participation and agency policies
Review documentation, OASIS accuracy, and quality outcomes
Conduct chart audits and provide meaningful coaching and education
Assist with hiring, onboarding, and staff development
Partner with physicians, hospitals, referral sources, and community partners
Monitor patient satisfaction, quality metrics, and regulatory compliance
Support clinicians with complex patient care needs
Help recruit and retain outstanding clinical talent

Qualifications

Current Illinois Registered Nurse (RN) license
Minimum of 2 years of Home Health experience required
Previous Clinical Manager, Clinical Supervisor, or leadership experience preferred
Strong knowledge of Medicare regulations, OASIS, and home health operations
Experience with Axxess EMR preferred
Excellent leadership, communication, organization, and problem-solving skills
Passion for mentoring others and building successful teams
