YourADH is a mission-driven healthcare organization dedicated to enhancing the health, independence, and quality of life of older adults and individuals with disabilities. Through our Adult Day Health program, we provide a supportive, engaging, and person-centered environment where participants receive coordinated nursing care, therapeutic services, social engagement, nutritious meals, and meaningful activities designed to promote overall well-being.

Our interdisciplinary team works closely with participants, caregivers, healthcare providers, and community partners to ensure each individual receives compassionate, personalized care while remaining active and connected within their community.

To support our continued growth and expand awareness of our Adult Day Health services, YourADH is seeking a Community Liaison who is passionate about community engagement, relationship building, and connecting individuals and families with programs that enhance independence, health, and quality of life.

The Opportunity

We are seeking a passionate, relationship-driven Community Liaison to support the growth and success of our Adult Day Health (ADH) program. This role serves as the primary ambassador for our organization within the community, building and maintaining referral relationships while guiding prospective participants and families through the admissions and enrollment process.

The Community Liaison plays a vital role in connecting older adults and individuals with disabilities to services that promote independence, health, socialization, and overall quality of life. This position combines community outreach, business development, admissions coordination, and relationship management to drive participant enrollment and strengthen community partnerships.

The ideal candidate is organized, self-motivated, personable, and committed to making a meaningful impact in the lives of older adults, caregivers, and community members.

Key Responsibilities

Community Outreach & Relationship Development

Develop and maintain strong referral relationships with hospitals, physician offices, skilled nursing facilities, social workers, case managers, elder service agencies, senior housing communities, and other community partners.
Conduct regular in-person outreach visits to referral sources throughout the service area.
Represent the organization at community events, health fairs, networking opportunities, educational programs, and outreach initiatives.
Deliver presentations, in-services, and educational sessions to healthcare providers, community organizations, and stakeholders.
Identify opportunities to expand referral networks and strengthen the organization's market presence.

Admissions & Intake Management

Manage the full intake and admissions process from initial referral through participant enrollment and admission.
Serve as the primary point of contact for prospective participants, caregivers, and referral sources throughout the enrollment process.
Coordinate intake activities including inquiry follow-up, scheduling assessments, collecting required documentation, and facilitating enrollment completion.
Conduct facility tours for prospective participants, families, referral partners, and community stakeholders.
Collaborate closely with clinical and operational teams to ensure smooth onboarding and successful admissions.

Marketing & Community Engagement

Support brand awareness initiatives through outreach campaigns, community partnerships, presentations, and marketing efforts.
Assist with the development, organization, and distribution of marketing and promotional materials.
Plan and participate in open houses, educational events, and community engagement activities designed to increase program visibility.
Maintain an active referral pipeline and track outreach efforts, referrals, follow-up activities, and outcomes.

Performance & Collaboration

Meet admissions, census growth, and qualified referral goals.
Monitor referral trends, market conditions, and competitive activity and provide feedback to leadership.
Collaborate with leadership to enhance outreach strategies and strengthen community recognition.
Participate in team meetings and organizational growth initiatives.
Perform other duties as assigned.

What We Are Looking For

Education

Bachelor's degree in Healthcare Administration, Human Services, Marketing, Business, Social Work, or a related field preferred.
Equivalent combination of education and relevant experience will be considered.

Experience

Minimum of 1–3 years of experience in healthcare outreach, community relations, admissions, business development, marketing, or related fields.
Experience working with older adults, caregivers, adult day health programs, home care, rehabilitation, senior living, healthcare organizations, or community-based services preferred.
Existing relationships within the local healthcare, senior services, or community services network are highly desirable.

Skills & Qualifications

Strong interpersonal, communication, networking, and relationship-building skills.
Ability to work independently while managing multiple priorities and deadlines.
Excellent organizational skills with strong attention to detail.
Customer-service focused with the ability to build trust and rapport with participants, families, and referral partners.
Proficiency with Microsoft Office and CRM/referral tracking systems.
Valid driver's license and reliable transportation required.
Bilingual candidates (English/Spanish or English/Cape Verdean Creole) are encouraged to apply.

Physical and Environmental Demands

Frequent local travel throughout the service area is required.
Ability to drive and travel independently to meetings, referral sources, and community events.
Regular walking, standing, and sitting throughout the workday.
Ability to conduct presentations, tours, and community outreach activities.
Work environment includes a combination of office-based administrative work and community-based field outreach.
May occasionally lift and carry marketing materials, event supplies, and presentation equipment up to 25 pounds.
Ability to communicate effectively in person, by phone, and electronically.
