Full Job Description

Full-Time Social Services Director (Skilled Nursing Facility)

A well-established Skilled Nursing Facility is seeking an experienced and compassionate Full-Time Social Services Director to lead our Social Services department. This leadership role is responsible for advocating for residents, supporting families, coordinating psychosocial services, and ensuring compliance with all federal and state regulations. The ideal candidate is organized, collaborative, and passionate about enhancing the quality of life for residents.

Key Responsibilities

Direct and oversee the Social Services department within the skilled nursing facility
Complete psychosocial assessments and develop individualized care plans
Provide counseling and emotional support to residents and their families
Coordinate admissions, discharges, and safe transition planning
Facilitate care conferences and collaborate with the interdisciplinary team
Assist residents and families with advance directives, community resources, insurance, and financial assistance referrals
Advocate for resident rights and ensure compliance with state and federal regulations
Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
Participate in quality improvement initiatives and survey readiness activities

Qualifications

Bachelor's degree in Social Work or a related human services field required (Master's degree preferred)
Current California Social Worker registration or licensure, if applicable
Minimum of two (2) years of experience in a skilled nursing, post-acute, or long-term care setting preferred
Previous leadership or supervisory experience preferred
Strong knowledge of Medicare, Medi-Cal, discharge planning, and long-term care regulations
Excellent communication, organizational, and interpersonal skills
Ability to build positive relationships with residents, families, staff, and community partners
