Full Job Description

Who is Bridging Access to Care, Inc.:Bridging Access to Care, Inc. (BAC) is a nonprofit, multi-service, community-based behavioral health care organization committed to building healthy communities, one person at a time.

In 2012, Brooklyn Aids Task Force changed its name to BAC and successfully made the transition from an AIDS-centered organization to a multi-service community-based healthcare organization. Brooklyn Aids Task Force was the very first HIV/AIDS organization in Brooklyn, prioritizing serving persons living with HIV/AIDS (PLWHA) since 1986. Today, BAC provides mental health and substance use-related social services, comprehensive HIV/AIDS care, and coordinated health care and supportive services to underserved racial and ethnic minority communities in Brooklyn and other parts of New York City.

JOB SUMMARY: As part of the DOHMH Young Adult Families (YAF) team, the Housing Specialist will empower young adults and families to achieve housing stability and greater independence. The Housing Specialist will work closely with case managers and community service providers, you will provide hands-on support to help participants develop the skills needed to maintain stable housing and transition toward more independent living.

In this role, the Housing Specialist will assist participants with housing searches, connect them to community resources, and promote practical life skills that support long-term self-sufficiency. Responsibilities includes but not limited to, conducting monthly apartment inspections, accompanying participants to appointments with agencies such as HRA and the Social Security Administration, monitoring monthly rent and utility payments to help prevent service interruptions, and addressing barriers that may impact housing stability. This position requires frequent field-based work, with up to 75% travel throughout the boroughs.

EDUCATION AND WORK EXPERIENCE:

MINIMUM: Bachelor’s degree in human services, social work, or related fields required
MINIMUM: Two (2) years of field experience providing housing and/or case management services to people experiencing homelessness, people with substance use disorder, and/or people with psychiatric illness
MINIMUM: One (1) year of continuous experience working as a housing specialist, or in a similar position providing housing placement services to people experiencing homelessness and/or living in transitional housing.

DUTIES AND RESPONSIBILITIES:

Client Assistance and Support

Meet with tenants on a monthly basis.
Conduct monthly apartment inspections to ensure tenants are living in an optimal environment.
Assess tenants’ daily living skills and provide individual and group skills development services as needed.
Assist tenants with their transition to independent housing.
Accompany tenants to appointments with HRA, Social Security, and other service providers to advocate for tenants’ rights and help them navigate these systems.
Collaborate with counselors and other service providers to deliver holistic support tailored to each family’s unique circumstances.
Provide referrals for various services as necessary.
Guide clients through the application process for affordable housing programs.

Housing Support

Complete initial, annual, and updated apartment inspections.
Assist clients in establishing utility services and monitor monthly payments to prevent service disconnection.
Resolve apartment repair and maintenance needs by working with BAC maintenance staff, building managers, and landlords. Build and maintain relationships with landlords and property managers to ensure all repairs are completed promptly.
Conduct workshops to assist participants in maintaining housing successfully and searching for independent housing in the community. Facilitate and co-facilitate tenant meetings and Independent Living Skills groups.
In collaboration with case managers, assist tenants in planning and transitioning into independent housing or other supportive housing facilities.
Ensure the documentation of service delivery is completed concurrently and collaboratively at the time of service delivery or within 24 hours of the encounter.

Program Coordination and Documentation

Maintain documentation including case conferences, progress notes, and all required correspondence.
Document all repair requests and outcomes in the tenant record.

Other Responsibilities

Perform other related duties as assigned, based on program needs.

KNOWLEDGE, SKILLS, AND ABILITIES:

Knowledge of issues related to substance use, recovery, and mental health.
Excellent computer skills, including proficiency in Microsoft Office (Word, Teams, Excel) and the ability to learn new programs.
Familiarity with electronic medical records (EMR) and client documentation.
Excellent written and verbal communication skills.
Strong problem-solving skills to address client challenges related to housing, employment, healthcare, and other barriers to independent living.
Familiarity with Medicaid, public benefits, and supportive housing programs.
·Ability to motivate and empower young adults facing complex challenges.
Ability to manage multiple cases simultaneously while maintaining attention to detail and professionalism.
