Job Summary

Monday thru Friday 10am to 5pm

Monday Thru Thursday 5pm to 9pm

Saturday and Sunday 10am to 7pm

Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized in-home support to individuals across various stages of life, including seniors, those with disabilities, and patients requiring end-of-life care. Your energetic and proactive approach will help promote independence, comfort, and well-being while adhering to home healthcare regulations and maintaining the highest standards of patient confidentiality. This paid position offers an opportunity to develop your caregiving skills in a rewarding environment that values your dedication and expertise.

Duties

Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
Monitor patient health by observing vital signs, recording medical information accurately in Electronic Health Record (EHR) systems, and reporting changes promptly
Support medication administration following prescribed protocols while ensuring compliance with HIPAA regulations to protect patient privacy
Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral management
Perform catheter care, wound care, and other medical procedures under the supervision of healthcare professionals
Help with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and comfortable living environment
Utilize patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility and prevent injury

Qualifications

Proven experience working as a Home Health Aide or in assisted living/nursing home settings with geriatric or disabled populations
Knowledge of medical terminology, basic life support (BLS), CPR, first aid, and home health care assistance techniques
Familiarity with EMR/EHR systems for documentation and medical records management
Ability to work independently while maintaining excellent communication with clients and healthcare teams
Experience working with individuals with developmental disabilities or in hospice care is preferred
Physical stamina for heavy lifting and assisting with patient transfers using methods like Hoyer lifts or patient transfer techniques
Valid certification as a Home Health Aide or comparable credential; additional certifications in dementia or hospice care are advantageous
Understanding of home & community care regulations and commitment to maintaining patient confidentiality under HIPAA guidelines
