Full Job Description

Position Summary

The Program Manager, Health Equity (HIV Prevention & Substance Use Services) is responsible for the development, implementation, coordination, and evaluation of TWOC Healing Project's HIV prevention, health education, harm reduction, substance use, and health equity initiatives within the Healing On Purpose Wellness Space (HOPWS). This position leads the expansion of community-based programming by cultivating strategic partnerships, strengthening collaborative relationships, and identifying opportunities that increase access to culturally responsive health services.

The Program Manager serves as the organization's subject matter expert on HIV prevention and substance use services, ensuring programming reflects current best practices, community priorities, and public health standards. Working closely with the Program Director and Executive Leadership, this position supports grant implementation, program development, community engagement, and continuous quality improvement through data-informed decision making and participant feedback.

This is a part-time position (up to 24 hours per week) with the expectation that hours and responsibilities will expand as funding opportunities and organizational growth permit.

Core Areas of Responsibility

1. Program Development & Management

The Program Manager develops, implements, and evaluates innovative health equity programming that advances TWOC Healing Project's mission and strategic priorities.

Responsibilities

Develop and implement HIV prevention, education, testing, linkage to care, and health promotion initiatives.

Develop substance use prevention, harm reduction, recovery support, and wellness programming.

Identify community needs and design responsive, evidence-informed services.

Develop annual work plans, program goals, and implementation timelines.

Coordinate workshops, trainings, educational events, and community wellness activities.

Ensure all programming is culturally responsive, trauma-informed, healing-centered, and aligned with organizational values.

2. Partnership Development & Community Collaboration

The Program Manager cultivates partnerships that strengthen health equity and expand access to care throughout the communities served by TWOC Healing Project.

Responsibilities

Develop and maintain partnerships with healthcare providers, behavioral health agencies, recovery organizations, public health departments, and community-based organizations.

Establish referral pathways that improve access to HIV prevention, treatment, recovery, and supportive services.

Represent TWOC Healing Project within local, regional, and statewide coalitions and advisory groups.

Identify opportunities for collaborative programming and shared funding initiatives.

Serve as the primary organizational liaison for HIV prevention and substance use partnerships.

3. Outreach, Education & Community Engagement

The Program Manager develops outreach strategies that increase awareness of available services while reducing barriers to care.

Responsibilities

Coordinate community outreach and educational campaigns.

Conduct presentations, trainings, and health education workshops.

Participate in community events, health fairs, and awareness activities.

Increase community awareness of HIV prevention, harm reduction, substance use resources, and available supportive services.

Promote equitable access to prevention, treatment, and recovery services.

4. Compliance, Reporting & Program Evaluation

The Program Manager ensures programs meet organizational and funding requirements while utilizing data to improve services.

Responsibilities

Maintain accurate program documentation and participant records.

Monitor program performance measures and key performance indicators.

Prepare monthly reports and assist with grant reporting requirements.

Collect, analyze, and interpret program data.

Develop participant feedback mechanisms to support continuous quality improvement.

Utilize evaluation findings to strengthen program effectiveness and community impact.

5. Organizational Leadership & Strategic Growth

The Program Manager contributes to the organization's long-term growth by identifying opportunities that expand health equity initiatives.

Responsibilities

Collaborate with leadership on strategic planning and program expansion.

Identify funding opportunities related to HIV prevention, behavioral health, and health equity.

Assist with grant development by providing programmatic expertise and supporting proposal development.

Collaborate with communications staff to promote health equity initiatives.

Participate in staff meetings, organizational trainings, and strategic planning activities.

Perform other duties as assigned.

Required Knowledge & Skills

Demonstrated knowledge of HIV prevention, HIV education, testing, linkage to care, PrEP navigation, and/or substance use prevention and harm reduction.

Experience developing and managing community-based health programs.

Knowledge of health equity, trauma-informed care, and culturally responsive service delivery.

Strong experience building partnerships and collaborative relationships across healthcare, public health, and community organizations.

Experience working with Black communities, LGBTQIA+ communities, and historically underserved populations.

Excellent communication, facilitation, and relationship-building skills.

Strong organizational, project management, and problem-solving abilities.

Experience with grant-funded programs, data collection, reporting, and program evaluation preferred.

Ability to work independently while managing multiple priorities.

Minimum Qualifications

Bachelor's degree in Public Health, Social Work, Human Services, Behavioral Health, Community Health, or a related field; equivalent experience may be considered.

Minimum of three (3) years of progressively responsible experience in HIV prevention, public health, behavioral health, substance use services, or community health programming.

Demonstrated experience developing partnerships and leading collaborative community initiatives.

Experience with program planning, implementation, and evaluation.

Valid driver's license and reliable transportation.
