Full Job Description

Education:

Master's in Social Work (Required)

General Duties:

Trains & Supervises five Case Workers
Reviews Case Workers case notes, psychosocials, etc.
Under the general direction of the Assistant Director, develops service plans for clients and provides guidance to Case Workers in implementing these plans to ensure court orders are adhered to.
Assists the Assistant Director and Program Director in developing and maintaining relationships with other programs and agencies providing services to Community Guardian clients.
Visits clients in the field along side Case Workers and/or carries a caseload in the absence of Case Worker(s) in the department, or when cases require more complex interventions.
Attends court hearings as requested.
Provides a monthly statistical report of clients visited, etc. by the staff that they supervise to the Assistant Director
Participates in regular team and staff meetings to coordinate client care.
On call on rotation for client emergencies/crisis intervention
Perform all tasks requested by the Assistant Director, Program Director and Director of Programs to support the team functioning of New York Foundation for Senior Citizens Guardian Services, Inc.
