Job Description

Please note: Ottawa County Community Mental Health is currently transitioning to an independent authority. As of January 1, 2027, this position will be a part of the Ottawa County Community Mental Health Authority.

Under the supervision of a Community Mental Health Program Coordinator, serves as a Supports Coordinator for adults with Intellectual and/or Developmental Disabilities.

This is a full-time, benefited position based out of the James Street Campus located at 12265 James Street, Holland, MI. Typical working hours are Monday - Friday, 8:00am - 5:00pm.

Essential Job Functions

The essential functions of this position include, but are not limited to, the following:

Receives referrals and conducts interviews to obtain necessary information to complete assessments of needs and community/natural support systems.
Develops plan of service with consumers and their chosen support system detailing the specific services needed to meet goals the consumer will participate in including health, personal development, school, work, relationship building, and community inclusion.
Chairs an interdisciplinary team composed of mental health professionals, friends and family chosen by the consumer to develop a plan of service and to provide and receive feedback on progress towards goals.
Monitors implementation of the plan of service for each assigned consumer. This includes providing regular follow-up, monitoring services related to plan goals, objectives, and planned interventions and completing periodic reviews, required assessments and status reports.
Assures adherence to all rules and regulations of the Department of Health and Human Services, CARF and the Habilitation Supports Waiver.
Coordinates services for consumers among all involved agency personnel as defined by the established treatment plan in areas such as housing, legal, clothing, medical/psychological care, social security, utilities, education, transportation, employment and recreation.
Enhances community understanding and acceptance of individuals with Intellectual and Developmental Disabilities and promotes mental health services through community presentations to civic and community agencies and other interested groups.
Uses a computer to maintain complete and detailed case records consistent with Department of Community Health standards, agency procedures and requirements of third-party funding sources.
Ensures that clinical documentation regarding consumers is completed as required and is kept secure and confidential and maintained consistent with Department of Community Health and Community Mental Health policies and procedures.
Complies with recipient rights reporting requirements established by the Community Health Code and procedures of the Community Health Services Board.
Assists respective supervisor(s) in evaluating the effectiveness of current programming and makes suggestions for improvements.
Other duties as assigned.

Required Education, Training and Experience

Bachelor’s degree in social work (BSW or LBSW) preferred; bachelor’s degrees in related human service fields (Psychology, Sociology, Counseling) accepted. One year of experience working with individuals with Intellectual/Developmental Disabilities is required. Relevant experience in a Community Mental Health agency is preferred. Community Mental Health work or other case management experience is highly desirable.

Certifications and Licensure:
Must meet Michigan qualifications as a Qualified Intellectual Disability Professional (QIDP). This includes a bachelor’s degree in human services field and at least one year of experience working with individuals with intellectual or developmental disabilities. Possession of a valid Michigan Bachelor’s Social Work license (LBSW, LLBSW) is preferred.

Possession of a valid Michigan driver’s license required.

Required Knowledge and Skills:

Good working knowledge of people with developmental disabilities and current supports and services coordination theory and practice for individuals with developmental disabilities.
Good working knowledge of the principles and practices of medical record documentation and medical records management. Ability to produce timely clinical notes.
Good working knowledge of community resources.
Understands and can apply a collaborative approach to both intra-agency and interagency relationships.
Ability to provide quality assessments of individuals.
Shows willingness to learn and use new skills and takes regular advantage of internal and external training and staff development opportunities and resources.
Knowledge-based competencies in person-centered planning.
Computer literacy and working knowledge of word-processing, spreadsheet, database and other necessary software programs.

Physical Requirements:
Must be able to access consumers at various locations.

Working Conditions:
Works in office locations and variety of community settings. Travels throughout the area to attend meetings and meet with consumers in the community and in their homes.
