Full Job Description

Who We Are:

Since 1972, The Hallmark has been the trusted name in upscale retirement living in Houston’s prestigious Galleria/Tanglewood area. As the oldest Life Plan Community in the region, we’re a not-for-profit community operated by Brazos Presbyterian Homes, Inc., dedicated to enhancing the lives of older adults of all faiths. We offer luxurious independent living, combined with a seamless continuum of care including assisted living, memory support, skilled nursing, and rehabilitation all on a single campus.

Who You Are:
We are seeking a candidate with a strong positive attitude toward customer service to act as a CNA in our Independent Living. This person will have a genuine interest in working with older adults and be able to engage, motivate, and support both residents and fellow team members. The ideal candidate is dependable, compassionate, and aligned with our values of respect, excellence, and community. Creativity, teamwork, and a drive to exceed expectations are key traits we’re looking for as we work together to fulfill our mission of uplifting the lives of those we serve.

Essential Functions:

1. Except as specifically noted, the following functions are considered essential to this position. Assistance may range from total to partial to stand by depending upon physical and cognitive ability of resident.
2. Assist residents in and out of bed and chair or wheelchair with the correct use of the transfer belt (gait belt). May require the assistance of another aide, depending on weight and physical abilities. May require the use of a mechanical lift to move the resident.
3. Assist residents in bathing as assigned.
4. Assist residents in dressing or undressing. Fold or hang clean items and place soiled items in hamper (family laundry) or lined barrel (facility laundry).
5. Assist residents in using bathroom and peri care. May be required to provide, remove and clean bedpans, urinals and bedside commodes.
6. Take vital signs of residents, e.g. temperature, blood pressure, pulse, and respiratory rate and record information.
7. Assist residents to dining area for meals or provide meals in room, as resident prefers. May be required to assist in tray setup and feeding of residents and documenting intake of nourishment. Serve between meal nourishment and assist as needed. Record amount eaten. Except when contraindicated, maintain clean pitcher and cup, and fresh water at bedside of each resident.
8. Remove soiled bedding and replace with clean linens as needed.
9. Assist in personal grooming of residents, e.g. brushing hair, teeth, trimming fingernails and toenails. Monitor hygiene continuously and assist as needed to assure that each resident is clean and neat.
10. If resident is unable to reposition self, aide may be required to turn and reposition resident intermittently.
11. May be required to clean equipment such as bedpans, basins and empty catheter bags.
12. Maintain each bedside unit in neat and orderly condition. Clean table surfaces as needed. Store personal items such as dentures, eyeglasses, combs, brushes in sanitary and convenient manner.
13. Make continuous observation of housekeeping and maintenance needs, e.g. broken fixtures, burned out light bulbs, soiled floors and curtains, and reports needs promptly.
14. Maintain bedside care supplies, e.g. gloves, tissues, toothpaste, use charge system to record supplies obtained from facility stock. Notify charge nurse to have them call if items are to be supplied by family.
15. Maintain all service areas in clean and orderly condition. This may include washing dishes, sweeping, mopping, and vacuuming floors, clean counter tops, and minor straightening of resident apartments.
16. Assist in assuring the maximum quality of life for all residents by maintaining privacy, accommodating individual preference, promoting independence, encouraging and assisting participation in facility activities and events and communication respectfully at all times.
17. Respond to all call lights promptly. Keep call light within reach of non-ambulatory resident when unattended.
18. Maintain continuous communication with team leader. Participate in report; communicate all changes and unusual observation concerning residents physical and psychosocial functioning.
19. Attend unit meetings as scheduled, all mandatory in-services and optional in-services per individual needs and interest, for a total of at least 12 hours annually. Outside educational offerings may be substituted for optional in-service hours by providing proof of attendance. Mandatory in-service cannot be substituted.
20. Remove soiled personal laundry, launder, fold, and put away in residents apartment.
21. Maintain work area and keep the department in compliance with OSHA safety standards.
22. Effectively communicates both verbally and in writing.
23. Demonstrates the community hospitality promises towards residents, families, associates and vendors.
