Full Job Description

General Summary

The Patient Care Manager is responsible for the overall direction of hospice clinical services. The Patient Care Manager establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. Patient Care Manager reports to Administrator.

Principal Duties and Responsibilities

1. Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel

2. Provides guidance and counseling to the clinical team to assist them in continually improving all aspects of hospice services, provided through organization personnel.

3. Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice care needs. Assigns appropriate hospice personnel to case as needed. Conferences with Medical Director regarding any questions about an individual’s eligibility for services.

4. Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course.

5. Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

6. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.

7. Provides help in assessment, planning, and implementation and evaluation of patient family/caregiver care to all clinical personnel as indicated.

8. Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees.

9. Assists the Administrator in the preparation and administration of the organization’s budget.

10. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues, or expenses.

11. Evaluates performance of the clinical team

12. Assists clinical team to develop skills and techniques in evaluating patients and outcomes

13. Hires, evaluates, and terminates clinical organization personnel.

14. Conducts annual evaluations on clinical personnel, or more frequently if indicated.

15. Oversees the quality of clinical entries in the patient medical record, statistics, reports, and records for purposes of evaluation and reporting of organization activities.

16. Assures proper maintenance of clinical records in compliance with local, state, and federal laws.

17. Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.

18. Develops, implements and evaluates the orientation program for new organization clinical personnel. Responsible for orientation of new organization clinical personnel, directly, or delegated.

19. Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel.

20. Assists with the evaluation of organizations performance vis performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the organization.

21. Assists in the development of organization goals. Develops, recommends, and administers organization policies and procedures.

22. Assures compliance with all local, state, and federal laws regarding licensure and certification of organization personnel and maintains compliance to the CHAP Hospice Care Standards.

23. Stays informed about changes in the field of nursing and hospice care, shares information with appropriate organization personnel.

24. Promotes hospice referrals in the health care community.

25. In the absence of the administrator, the Patient Care Manager will become the acting Administrator and will be vested with the authority to act in behalf of the administrator.

26. Other duties as assigned by the Administrator.

Required Skills

1. Registered nurse in the State with the current licensure to practice professional nursing in the State.

2. Complies and maintains current CPR certification.

3. Previous experience in hospice/home care settings and with two (2) years management or supervisory experience. Proven ability to work within an interdisciplinary setting.

4. Demonstrated ability to supervise and direct professional and administrative personnel.

5. Understands principles of pain/symptom management.

6. Ability to market and deal tactfully with customers and the community.

7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

8. Has excellent observation, verbal, and written communication skills.

9. Knowledge of business management, government regulations and CHAP standard
