Position Summary

The Care Team Manager (RN) is a clinical leadership role responsible for overseeing quality hospice care delivery, supporting field clinicians, and ensuring compliance with CMS Hospice Conditions of Participation (CoPs), state regulations, and agency standards.

This role is designed for an experienced hospice nurse leader who is passionate about both patient care and operational excellence. During the growth phase of our Northwest Suburbs and Northshore territory, this position will initially function in a dual-capacity role that includes both direct patient case management and clinical leadership responsibilities. As the territory expands, the position will transition into a primarily leadership-focused Care Team Manager role with direct patient visits provided periodically as needed for patient coverage, staffing support, admissions assistance, or high-acuity situations.

The Care Team Manager serves as a clinical resource, mentor, and operational leader for the interdisciplinary team and plays a key role in maintaining survey readiness, documentation excellence, staff accountability, and quality outcomes.

Training for this role will take place in Oakbrook Terrace, IL and surrounding service areas. The primary territory for this position will support the Northwest Suburbs and Northshore regions, with initial office days based in Oakbrook Terrace until a northern territory office location is established.

Key ResponsibilitiesClinical Leadership & Team Management

Provide day-to-day clinical leadership and oversight for field RNs, LPNs, and hospice aides.
Foster a culture of accountability, teamwork, professionalism, and compassionate patient-centered care.
Serve as a mentor and clinical resource to staff regarding hospice regulations, symptom management, documentation standards, and patient care best practices.
Assist with orientation, onboarding, coaching, training, and ongoing development of clinical team members.
Partner with executive and clinical leadership on staffing, workflow optimization, operational initiatives, and strategic growth.
Support interdisciplinary team collaboration to ensure continuity and excellence in patient care delivery.

Regulatory Compliance & Quality Oversight

Ensure compliance with CMS Hospice Conditions of Participation (CoPs), IDPH requirements, agency policies, and hospice best practices.
Maintain a strong focus on survey readiness, clinical quality, and documentation accuracy.
Conduct routine chart audits and quality record reviews to ensure timeliness, compliance, and appropriateness of documentation.
Monitor visit frequency, care plan follow-through, physician orders, recertification readiness, and interdisciplinary documentation standards.
Identify trends, compliance risks, and opportunities for process improvement and staff education.
Participate in QAPI and performance improvement initiatives to support exceptional patient outcomes and operational excellence.
Assist leadership with implementation and monitoring of clinical quality initiatives and corrective action plans when needed.

Patient Care & Clinical Support

Coordinate and manage care for an assigned group of hospice patients during territory growth and staffing support periods.
Complete comprehensive nursing assessments and participate in development and oversight of individualized plans of care.
Collaborate closely with physicians, facility staff, families, and the interdisciplinary team to ensure continuity and quality of care.
Provide patient visits and field support periodically as needed for staffing coverage, admissions, urgent clinical situations, or operational support.
Provide education and emotional support to patients, families, and caregivers.
Participate in on-call rotation as assigned.

Qualifications

Current Registered Nurse (RN) license in the State of Illinois.
Minimum of 2 years of hospice experience required; leadership, supervisory, QA, or clinical management experience strongly preferred.
Strong knowledge of Medicare Hospice Conditions of Participation (CoPs), hospice documentation standards, and regulatory compliance requirements.
Demonstrated leadership, mentoring, organizational, and critical thinking skills.
Experience conducting chart audits, quality reviews, and clinical oversight preferred.
Proficiency with EMR documentation and hospice clinical workflows.
Excellent communication and interpersonal skills.
Ability to manage emotionally sensitive situations with professionalism, empathy, and confidence.
Willingness to travel throughout the assigned territory for patient visits, facility support, and staff oversight.
Ability to lift and/or assist in moving patients when necessary.
Valid driver’s license, reliable transportation, and current auto insurance required.
