Care Manager with a focus on Care Solutions – Clinical Healthcare Background

CayCare is looking for a Placement Navigator to join our team.

For more than 20 years, CayCare has helped families navigate some of the most stressful moments of their lives—hospital discharges, rehabilitation transitions, changing care needs, and the search for the right senior living or care environment.

Our Placement Navigators meet families when they are often overwhelmed and asking one very important question:

“What do we do next?”

Your job is to help them answer it.

What You’ll Do

As a Placement Navigator, you will work independently in the community helping older adults and their families understand their care options and move toward an appropriate placement.

You will:

Meet with patients and families in hospitals, skilled nursing facilities, homes, and other healthcare settings
Assess the family's situation, needs, preferences, timeline, and available resources
Educate families about senior living and long-term care options
Help families navigate Adult Family Homes, Assisted Living, Memory Care, and other appropriate resources
Communicate with healthcare professionals and community partners
Coordinate tours, referrals, and placement options
Follow families through the transition rather than simply handing them a list
Maintain timely and professional documentation
Travel throughout the assigned service area
Manage your schedule and workload with a high degree of independence

Who We’re Looking For

This position is best suited for someone with a clinical healthcare background who also genuinely enjoys talking with people.

Clinically knowledgeable and able to understand medical terminology and care needs

Comfortable building relationships with hospitals, rehabilitation facilities, senior communities, and other healthcare professionals
Excited by a position that includes both flexibility and travel
Able to manage changing priorities without needing constant supervision

Healthcare experience is important. Personality is equally important.

We need someone who can understand the clinical picture while also sitting across from a worried spouse, son, or daughter and helping them feel that there is finally a plan.
