Full Job Description

Full Time Case Manager-SUD
Our OC Mental Health and Addiction Treatment Center is seeking a full time case manager to join our team of health care professionals. This position will be Monday-Friday.

The Case Manager Role

The Case Manager is responsible for the support and mentoring of clients. He/She will maintain a caseload. Acts as the central receiver of pertinent clinical data and works closely with the Clinical Lead in the formulation of treatment planning.

Essential Duties and Responsibilities

-Treatment/Recovery Planning

-Documentation of progress and treatment plan reviews

-Discharge planning and documentation

-Clinical updates to family and/or referral sources (as allowed via client consents).

-Regular sessions with clients, as outlined via Participant Admission Agreement and Treatment Plan.

-Crisis Intervention

-Referral to outside providers (dental, legal, psychiatry etc.)

-Group facilitation; both process and educational groups

-Preparation of group materials and researching new methodologies for inclusion in group and -individual sessions

-Provides group counseling services and treatment planning for individuals with alcohol and drug abuse issues through various methods including psycho-education and group counseling.

-Provides clients individual counseling, case management services.

-Provides crisis intervention.

-Creates treatment plan and individual schedule for each client.

-Enforces rules of the facility and implements consequences when needed.

-Maintains client chart and files all notes, records for their clients.

-Ensures client progress and updates treatment plan as needed.

-Coordinate continuum of care planning – working with clinical team, admissions team and client’s family.

-Must have an understanding of client confidentiality and HIPAA

-Requires after hours and weekend work and assigned holidays as needed for adequate client care.

-Responsible for performance improvement program in support of the Joint Commission

Minimum Qualifications (Knowledge, Skills, and Abilities)

-2 YEARS of experience as a Case Manager with strong professional references attesting to high -degree of clinical sophistication.

-Valid CCAPP Certification/CDAC-1 or RADT

-Knowledge of treatment and referral community in Southern California.

-Current certification in First Aid/CPR

-Valid California Driver’s license
