Full Job Description

Join Our Rehabilitation Team!

We are seeking a compassionate, motivated, and patient-focused Physical Therapist (PT) to join our interdisciplinary rehabilitation team. Our skilled nursing facility provides short-term rehabilitation, long-term care, and post-acute services with the goal of maximizing each resident's independence, mobility, and quality of life.

The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions while collaborating with physicians, nursing staff, occupational therapists, speech-language pathologists, and families to achieve optimal patient outcomes.

Essential Job Responsibilities

Perform comprehensive physical therapy evaluations and establish individualized plans of care.
Develop measurable functional goals based on resident needs and physician orders.
Provide skilled therapeutic interventions including therapeutic exercise, gait training, balance training, transfer training, neuromuscular re-education, manual therapy, pain management, and functional mobility training.
Evaluate and recommend adaptive equipment, mobility devices, and fall prevention strategies.
Monitor resident progress and modify treatment plans as clinically appropriate.
Complete timely, accurate documentation in the electronic medical record in accordance with Medicare, Medicaid, managed care, and facility requirements.
Participate in interdisciplinary care plan meetings, discharge planning, and family education.
Collaborate with rehabilitation staff, nursing, physicians, and other healthcare professionals to ensure coordinated resident care.
Educate residents, families, and caregivers on home exercise programs and safe mobility techniques.
Maintain compliance with all federal, state, facility, and rehabilitation regulations.
Assist with quality improvement initiatives and maintain productivity standards established by the rehabilitation department.

Qualifications

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program.
Current Ohio Physical Therapist license in good standing.
Strong clinical assessment, communication, and documentation skills.
Ability to work independently and as part of an interdisciplinary team.
Experience in skilled nursing, post-acute rehabilitation, geriatric care, or long-term care preferred but new graduates are encouraged to apply.

Preferred Experience

Skilled Nursing Facility (SNF) experience.
Knowledge of Medicare Part A, Part B, Medicaid, Managed Care, and PDPM documentation.
Experience using electronic medical records (EMR).
Knowledge of fall prevention, orthopedic, neurological, and cardiopulmonary rehabilitation.

Physical Requirements

Ability to stand, walk, bend, kneel, and assist residents throughout the workday.
Ability to safely transfer, lift, position, and assist residents using appropriate body mechanics and mechanical lifting equipment.
Ability to lift up to 50 pounds with assistance when necessary.
