Requirements:

Case Manager Position Summary:
The Special Education Case Manager coordinates special education services to ensure students with disabilities receive high-quality instruction, individualized supports, and a free appropriate public education (FAPE) in the least restrictive environment. This position oversees compliance with federal and state special education regulations while collaborating with teachers, related service providers, families, and administrators to support student success.

This role requires exceptional organization, proactive communication, attention to detail, and a commitment to ensuring students receive
appropriate services while maintaining full compliance with IDEA, ISBE regulations, and school procedures.

Case Manager Core Expectations:

Maintain full compliance with all federal, state, and local special education requirements.
Serve as the primary case manager for an assigned caseload of students.
Collaborate effectively with teachers, families, related service providers, and administrators.
Maintain accurate, organized, and confidential student records.
Exercise initiative, professionalism, and sound judgment while advocating for students.

Case Manager Key Responsibilities:

Case Management

Serve as the primary case manager for an assigned caseload of students with disabilities.
Develop, schedule, facilitate, and monitor annual reviews, reevaluations, eligibility meetings, manifestation determinations, and transition meetings.
Ensure all IEPs, evaluations, progress reports, and related documentation are completed accurately and within required timelines.
Maintain compliant special education records and student files.
Coordinate implementation of accommodations, modifications, and related services across instructional settings.

Compliance

Ensure compliance with IDEA, Section 504, Illinois School Code, and ISBE regulations.
Monitor implementation of IEP services and accommodations.
Maintain accurate service logs and documentation.
Coordinate evaluation timelines and required documentation.
Assist the Director of Special Education with compliance monitoring, audits, and state reporting requirements.

Student Support & MTSS

Participate in the school's tri-weekly MTSS meetings to review student progress, coordinate interventions, monitor implementation of IEP services, and collaborate with grade-level teams to ensure students with disabilities receive appropriate academic, behavioral, and

social-emotional supports.

Monitor student progress toward IEP goals and collaborate with instructional staff to adjust supports as needed.
Coordinate interventions and progress monitoring within the school's MTSS framework.
Support transition planning and postsecondary readiness for students receiving special education services.
Ensure students receive appropriate accommodations during classroom instruction and state assessments.

Collaboration

Collaborate with general education teachers to support implementation of accommodations and specially designed instruction.
Consult with teachers regarding instructional strategies and student supports.
Coordinate services with related service providers including speech-language pathologists, school psychologists, social workers, occupational therapists, and other specialists.
Maintain ongoing communication with parents and guardians regarding student progress and services.
Serve as a liaison between families, school personnel, and outside agencies when appropriate.

Professional Responsibilities

Participate in IEP meetings, professional learning, school improvement initiatives, and staff meetings.
Maintain current knowledge of special education laws, instructional best practices, and compliance requirements.
Support a collaborative culture focused on student achievement and continuous improvement.
Perform other duties as assigned by the Director of Special Education or Principal.

Case Manager Professional Expectations:
This position requires exceptional organization, initiative, collaboration, and attention to detail. Staff are expected to consistently meet compliance timelines, maintain accurate documentation, communicate professionally with families and staff, and advocate for the success of every student receiving special education services.

Case Manager Qualifications:

Required:

Professional Educator License (PEL)
LBS1 Endorsement
Knowledge of IDEA, Section 504, Illinois School Code, and ISBE special education requirements
Strong organizational, communication, and collaboration skills

Preferred:

Experience serving as a Special Education Case Manager
Experience facilitating IEP meetings
Experience working in a high school setting
Knowledge of MTSS and inclusive instructional practices

This position reports directly to the Director of Special Education & Principal.
